May 29-During a clothed body search pat down an inmate suddenly turned and struck the shoulder of the officer conducting the search with his elbow, the inmate attempted to f lee but was forced to the ground by two officers. Once on the ground, the inmate continued to resist but was eventually restrained and escorted.

May 26- During an interview with an LCSW, an inmate threatened to kill a Custody Staff Member by unspecified means. The inmate submitted to restraints and was removed from his cell and re-housed in Administrative Segregation without incident.
 
Also on May 26- At approximately 1313 hours, two Northern Hispanic inmates attacked three Southern Hispanic inmates who were being released from their assigned cells. OC and Exact Impact rounds were used to stop the threat. All involved inmates were removed from the affected area and allowed to decontaminate. No staff was injured as a result of the incident.
 
May 25- While an inmate was being escorted to the CTC for an interview with a psychiatr ist, he began rubbing his genitals while staring at a female officer. The officer ordered him to stop his actions and he complied but said he didn’t know what the female officer was talking about. The inmate was placed in hand restraints and was placed under Indecent Exposure Precautions for 90 days.
 
Also on May 25- An inmate expressed a threat to a staff psychiatrist towards a female officer. The inmate threatened to gas the officer. The officer was  interviewed and advised that a threat assessment would be submitted for review.
 
Also on May 25- Two officers were assaulted while escorting an inmate from the t reatment center to the housing uni t. The officers used physical force to get the inmate to the ground. One of the officers was pulled to the ground as she was attempting to subdue the inmate. She was medically evaluated and suffered a strained hamstring muscle.
 
May 21-An inmate committed the act of Sexual Misconduct-Masturbation With Exposure. The inmate exposed himself to an officer and continued to reposition himself in his cell to ensure that the officer could see him. The inmate had additional securi ty precautions placed on him and all appropriate administrative staff were notified of the incident.
 
May 20- At approximately 1825 hours, an inmate intentionally exposed and masturbated himself to a psychiatric technician during an evening group session. All indecent exposure restrictions were placed on the inmate.
 
May 19- An inmate intentionally head butted an officer in the center of his forehead while being escorted to a psychiatr ic appointment. The officer utilized physical force to take the inmate to the f loor of the rotunda. Responding staff ar rived on the scene and assisted the assaulted officer. The officer head butted received bruising to the forehead and was t ransported to an outside hospital.
 
Also on May 19- During shower program, an inmate was ordered to kneel for the placement of leg restraints and he complied. As the officer attempted to place the restraints on the inmate he was kicked on the right shin. Force was utilized to get the inmate into a prone position. No fur ther injuries to staff were reported.
 
Also on May 19- An inmate became resistive to staff orders and attempted to break escort from the officers. Physical force was used to stop the inmate’s actions and to take him to the f loor. There were no injuries to staff reported in the incident.
 
May 18- An inmate began covering his cell front with pieces of magazine pages. Staff utilized one burst of OC spray to get the inmate to comply with officers. Once the inmate submitted to mechanical restraints he was removed from the cell and placed in a medical interview cell to confer with medical staff. No staff were injured in the incident.
 
May 12- A female officer observed an inmate standing in front of his cell window masturbating while staring at her. The inmate was placed on 90day placard cell covering status.
 
May 5- An inmate commit ted battery on two officers. One of the officers hadknelt down to apply leg irons on an inmate, however, as the cell door was opening, the inmate jumped up and kicked the officer in the knee. Another officer assisted the officer that was struck in the knee and received an injury to his left arm in the process. The officer struck suffered redness, scratches, and bruising to both shins and swelling to his r ight knee and the other officer suffered redness and bruising to his left upper arm.
 
Also on May 5- An inmate willfully resisted staff while under escort to receive his medication. Officers used physical force to subdue the inmate and overcome his attempt to break away. The inmate eventually received his medication and was returned to his cell without fur ther incident.
 
May 1-An inmate became disruptive during contraband watch in the CTC and began banging his hands and head on the cell door. Medical staff declared that due to the inmates self-injur ious state of mind and refusal to take his medication, immediate force should be used to get him under control and given an injection. The injection was part of the inmate’s ordered medication. The inmate became combative and lunged at staff with his body. Staff utilized physical force and took him to the ground. The inmate stopped resisting staff and medical staff was able to administer the medications.
 
Also on May 1- An inmate on contraband surveillance watch began banging his hand and kicking the observation cell door. Staff gave him several orders to stop his disruptive behavior and finally utilized two bursts of OC to get the inmate to comply and desist in his behavior.
