April 2012

 

April 20 – While officers were conducting the evening medication pass, an inmate spun around forcefully pulling the handcuff into the food port. One officer gave verbal orders to relinquish the handcuffs with negative results. The inmate then began to pull the handcuff and the officer’s hand inside the food port, causing the officer to strike his hand against the food port. The other officer ordered the inmate to stop or pepper spray would be utilized, which the inmate did not comply. Pepper spray was used with negative results as the inmate continued to pull on the handcuffs. Pepper spray was used again as the officer’s hand was forcefully pinned between the handcuff and the food port opening. The officer attempted to use his hand to break free but was unable to do so because of the tension being pulled by the inmate. Pepper spray was used again with positive results.

 

April 16 – An inmate refused his medication so a cell extraction team was assembled. As the extraction team entered the cell the inmate kicked the shield, hit an officer’s chest with a closed fist and struck another officer’s chin, abdomen and back. The extraction team used physical strength to stop the inmate’s actions and roll the inmate over onto his stomach. Physical strength was used to place handcuff and leg irons on the inmate. An officer sustained pain to the hip and knee.

 

April 13 – As an officer was placing leg irons on an inmate, the inmate went forward landing on his side and kicking back with both feet, striking the officer’s knee. Another officer activated his personal alarm. The officer maintained control of the inmate until an RN arrived to administer medication. An officer sustained redness to the knee and another officer sustained pain and swelling to the chin.

 

April 12 – During an escort an inmate turned and kicked the escorting officer in the knee. The officer maintained control of the inmate using physical strength and holds to bring the inmate to a prone position. Responding staff arrived, placed the inmate in leg irons and escorted him to a holding cell. The officer sustained abrasions and pain to the hand and pain to the knee.

 

Also on April 12 – During an escort an inmate kicked an officer striking him on the ankle. The escorting officers using physical strength to bring the inmate to a prone position.

 

April 8 –An officer used pepper spray to stop an inmate’s attack on a Licensed Psychiatric Technician.

 

April 7 – During an escort an inmate used his body weight to fall to the floor. Escorting officers ordered the inmate to get up with negative results. The inmate then began forcefully thrashing his body from side to side in an attempt to break from the officer’s grip. The officer continued to maintain control of the inmate’s arm and ordered the inmate to stop his actions. Another officer arrived and sounded her personal alarm and assisted the officer by utilizing her body weight to keep the inmate in a prone position. The inmate was put on a gurney and escorted to TTA. During the escort the inmate threatened to bite the officer and deliberately lunged forward toward the officer and attempted to bite his hand. The officer was able to block the attack, using physical force to place the inmate back on the gurney.

 

Also on April 7 – An officer was conducting medication pass and tray pickup when he approached a cell. The officer went to grab the tray off the food port and the inmate grabbed the tray with a milk carton on it and threw it at the officer. The carton contained an unknown substance which struck the officer’s thigh and hand. The officer secured the food port and notified his sergeant.

 

April 1 – During an escort an inmate suddenly turned and lunged head first at one of the escorting officers in an attempt to head butt him. The officer used physical force to bring the inmate to a prone position. The other escorting officer sounded his personal alarm and maintained control of the inmate’s legs. The officers applied wrist restraints until responding staff arrived.

March 2012

 

March 26 – An officer was assisting two other officers as they conducted clothed body searches as inmates were exiting A7 Block. An inmate exited the yard door and without warning punched the officer striking him on his face. The other officers used physical force to bring the inmate to the ground. A unit alarm was activated and the inmate was placed in leg irons and a spit mask. The officer sustained abrasions with pain and bruising under his eye. The officer was transported to an outside hospital for further medical evaluation.
 
March 24 – As an inmate was thrashing violently, officers were responding to control the inmate. As one of the officers grabbed the inmate’s foot, the leg restraint chain became wrapped around the officer’s hand. The officer screamed out in pain and the inmate realizing the cause of pain continued to increase his violent movement to further the officer’s pain. The officer was eventually able to free his hand. The officer was seen by medical staff and referred to an outside hospital for further treatment of the injury to his wrist.
 
March 21 – As an officer opened the food port, an inmate shoved his arm through attempting to grab the officer. The officer stepped back and drew his pepper spray. The officer ordered the inmate to pull his arm back into the cell with negative results. A sergeant then ordered the inmate to remove his arm and warned the inmate that pepper spray would be used. The inmate did not comply so the sergeant utilized pepper spray with negative results. The officer then used a Fogger and deployed the spray through the crack of the door. The inmate then pulled his arm back through which allowed the securing of the food port.
 
March 20 – An inmate kicked and struck an officer on his shin. The officer maintained his grasp and brought the inmate to the ground. The inmate continued to resist and the officer activated his personal alarm. The inmate continued to resist so the inmate was placed in a spit mask and a retention chain.
 
March 19 – Officers were issuing the morning meal when an inmate grabbed a tray and immediately threw it back at the officer. The inmate then attempted to grab the officer. The
officer then utilized pepper spray to stop the inmate’s actions.
 
March 14 – Staff were conducting their duties when an inmate exited his cell for a shower to which staff would not let him pass an item to an inmate down the tier. The inmate spun around and lunged at the officer striking him in the head. The officer was slightly disoriented from the impact and used physical force to bring the inmate to a prone position. The Control Officer activated his personal alarm. Responding staff applied a spit net and the inmate was escorted to a holding cell. The officer sustained redness and swelling to his forehead, pain on the elbow and shoulder.
 
Also on March 14 – Staff were picking up food trays from the evening meal when an inmate threw a milk carton full of an unknown liquid substance at the officer through the food port. The liquid penetrated the officer’s jumpsuit making contact with the officer’s leg.
 
March 13 – Officers responded to an inmate kicking a group room door inside the dining room. Due to the inmate’s agitation, officers began escorting the inmate back to his cell. During the escort the inmate kicked a stack of bread racks. Officer used physical force to bring the inmate to a prone position. One of the officers activated his personal alarm and the inmate was placed in leg restraints and a spit mask. Responding officers then began to escort the inmate who was still resistive. The inmate then kicked one of the officers. Physical force was used to stop the inmate’s actions.
 
Also on March 13 – Staff were conducting the evening medication pass when an inmate refused proper compliance with the DOT. The inmate spun towards an officer and attempted to head butt him. Officers used their combined strength and weight to force the inmate to the ground. Responding staff applied leg restraints and spit mask. One of the officers sustained redness and abrasions to his knees.
 
March 8 – An inmate spit his medication and water back at staff, striking the LPT in the face and shirt.
 
March 7 – An inmate was agitated and yelling while repeatedly threatening staff. An officer ordered the inmate to stop. As the officer walked by the inmate, the inmate leaned over and spit through the holding cell attempting to strike the officer.
February 2012

 

February 29 – Two officers were escorting an inmate when the inmate attempted to head butt one of the officers. The inmate then kicked backwards twice, striking an officer in the lower leg. The inmate was placed in a holding cell, where he proceeded to take all of his clothes off and urinate inside the cell.
 
February 26 – An inmate violently jerked his body using his head to strike an officer. The officer used physical strength and body weight to push the inmate to the ground in a prone position. The officer sustained redness and bruising to the forearm. Following treatment the officer returned to full duty.
 
February 21 – An inmate became belligerent and began yelling at an LVN. As the inmate would not submit to handcuffs, an officer attempted to use physical force to place the inmate’s arm behind his back. The inmate resisted by flexing his arm and trying to pull his hand free. The officer was unable to force the inmate’s arm behind his back and the inmate pushed against the officer’s chest, causing the officer to fall against the handrail of the lower stairs. The inmate then dropped to his knees and wrapped his arms around the officer’s thighs. An alarm was activated and responding staff placed the inmate in handcuffs. The officer sustained an abrasion to the hand.
 
February 18 – An officer and sergeant were escorting an inmate when the inmate kicked his leg back, striking the officer’s knee. The officer used physical strength and holds, forcing the inmate to the ground. The officer sustained pain to the knee.
 
February 10 – An officer ordered an inmate to comply to an unclothed body search prior to being removed from his cell. The inmate refused to comply and became argumentative. As the officer started to close the food port the inmate struck the food port door causing it to strike the officer’s hand. The officer activated his personal alarm, at which time the inmate swung his fist out of the food port and struck the officer’s duty belt. The officer blocked the attack and took hold of the inmate’s hand. The inmate then grabbed the officer’s wrist and refused to let go. A responding officer used pepper spray with positive results. The officer sustained redness to the hand. The officer was treated on site and returned to his assigned duties.
 
February 5 – While staff were escorting an inmate, the inmate lunged towards an officer, striking his chest and shoulder. The officer grabbed the inmate as he was continuing his assault. Another officer activated his personal alarm as the first officer was continuing to stop the inmate’s actions. A responding officer gave a verbal order to get down and the inmate refused. The officer then used pepper spray with positive results. The responding officer applied leg restraints and placed the inmate in a holding cell. One of the responding officers sustained pain and swelling to the knee and was transported to an outside hospital.
 
February 3 – An inmate was observed masturbating during group and was asked to exit the room. The LPT reported the situation to an officer. The inmate became loud and produced an Inmate Appeals Form. The officer ordered the inmate to turn around and submit to handcuffs but the inmate would not comply. The officer ordered the inmate a second and third time to submit to handcuffs with negative results. As the officer attempted to place cuffs on the inmate, the inmate pulled away. Three officers then attempted to restrain the inmate but he continued to resist. The officers used their combined body weight, forcing the inmate to the floor. Once on the floor the inmate began kicking and swinging his arms wildly. An officer maintained control of the inmate as a sergeant placed handcuffs on the inmate. The inmate then attempted to grab the sergeant’s testicles but instead grabbed the inner thigh. In response to the pain, the sergeant used physical force to stop the inmate’s actions. One officer sustained scratches and redness to his hand. The sergeant sustained bruising to his inner thigh.
January 2012

 

January 20 – As officers arrived at the cell door following the inmate’s act of indecent exposure, the inmate began to flex up and take a bladed stance toward one of the officers. The officer advised the inmate to submit to handcuffs. The inmate refused to comply with staff orders, becoming agitated and moving toward the officer. The officer used pepper spray striking the inmate in the face. The Control Booth officer sounded the unit alarm. The inmate immediately rushed the officer striking him on the face. The other officer used pepper spray striking the inmate on the body and face. The officer then used his baton, striking the inmate who was still attacking the first officer. The inmate then turned and lunged toward the second officer. The officer used physical force to bring the inmate to the ground. The inmate was placed in hand cuffs and leg restraints. The first officer sustained abrasions, redness and swelling to the neck and face. Due to the injuries to the head, medical staff requested the officer be sent to an outside hospital for further evaluation and treatment. The second officer sustained swollen areas to the face and ear and exposure of pepper spray. The second officer was also sent to an outside medical facility for further treatment. 
 
Also on January 20 – An inmate committed Battery on a Peace Officer with a Weapon resulting in serious bodily injury which required the use of physical force. The inmate, who is a known case of Hepatitis C, bit an officer on the hand. 
 
January 14 – An officer was escorting an inmate back to his assigned cell. As the officer was closing the cell door the inmate kicked the officer striking him on the hand. The officer along with a responding officer took control of the inmate and ordered the inmate back out of the cell. The inmate twisted his body attempting to break free from the officers’ grasp. The officers used force to bring the inmate to a prone position. The inmate was placed in leg irons and escorted to a holding cell. The initial escorting officer sustained a bruise to his hand. 
December 2011

December 7 – A riot broke out involving 50 inmates. Rioters stabbed two inmates during the fighting. Staff used pepper spray, rubber projectiles and also fired shots to stop the riot. Several staff members were treated for minor injuries. 

April 2011

April 24 – During an escort, the inmate kicked the escorting officer on the shin. The officer began to fall to the ground and pulled the inmate towards the ground with him in an attempt to maintain control of the inmate. Responding staff assisted by grabbing the inmate and pushing him to the ground while maintaining control of the inmate. Staff applied physical holds to maintain control of the inmate while he was placed in restraints. The battered officer sustained
scratches and redness to the shin.


April 23 – While processing a visitor an officer detected the odor of marijuana emanating from the visitor. Eventually the visitor voluntarily relinquished two balloons of marijuana weighing approximately 67.1 grams.


April 22 – An officer was collecting urine samples from inmates when an inmate threw the urine sample at the officer striking him on his legs and groin area.


April 19 – An inmate threw coffee from his morning meal on the ground and struck an officer on the leg. The officer used physical force to place the inmate on his bed to prevent him from committing a second battery. The inmate then began kicking his legs, striking the officer on his chest and face. The inmate continued to resist the officer. Responding staff activated their personal alarm and used physical force to gain compliance. The officer sustained a kick to the chest and an abrasion to the face.


April 4 – Officers were collecting trays from the morning meal when an inmate reached through a tray slot with a milk carton throwing the unknown liquid contents at the officers. The liquid struck the officers in the face and forearm. The affected staff secured the slot and exited the tier. A sample of the liquid smelled of urine. The officers washed their exposed skin and returned to work without further incident.

February 2011

February 8 – Staff were processing an inmate from the block to the main yard when an inmate triggered the metal detector wand. The inmate tried to flee from staff attempting to handcuff him.
Four other inmates began moving towards officers. One of the approaching inmates struck a sergeant several times in the side of his face and jaw. The inmate began running away but the sergeant drew his baton and pursued the inmate and immediately got the inmate in a prone position. As the initial inmate began running, two officers tried to grab a hold of the inmate to prevent him from retrieving something from his waistband. The inmate was swinging his fists at the officers. Nine other inmates started to move toward the incident. An officer moved in between the inmates and the officers who were struggling with the inmate, ordering them to back up. The officer was then punched in the face and back of the head and fell to the ground where he was kicked and punched approximately three or four times in the back and shoulder areas. The officer forced himself back up to his feet at which time he pushed two inmates off of him as a third inmate backed away. The officer pursued the inmate with his baton drawn and ordered the inmate to get down and he complied. At this time the initial inmate spun around and struck an officer on the cheek. The officer grabbed the inmate and forced him to the ground utilizing his strength and weight. Another officer saw two inmates running toward the officer, so the officer used pepper spray. The initial inmate continued to struggle while on the ground and struck the officer two more times in the head and facial area. All inmates were eventually handcuffed. One inmate-manufactured weapon was recovered. Four officers were transported to an outside hospital for treatment of their injuries.
 

August 2010

August 28 – Two officers were escorting an inmate back to his cell when the inmate punched one of the officers. Both officers used strength and body weight to stop the inmate’s actions. One officers sustained soreness in the shoulder and the other sustained pain in both arms and tingling of the hand and was subsequently transported to an outside medical facility for further evaluation.

July 2010

July 22 – During the morning food tray pickup, an officer opened the cell tray slot and was subsequently gassed with a clear liquid that hit the officer’s leg. The officer was medically evaluated and remained on duty.

July 20 – An inmate threw water on an officer.

July 16 – During an escort an inmate became physically combative with staff. The inmate was forced to the ground by responding staff. Three officers were taken to an outside hospital for treatment.
 

June 2010

June 12 – An officer and an LVN were conducting Direct Observation Therapy Medication Pass when an inmate began to dispute a cell move with the officer. Without warning, the inmate punched the officer in the jaw. The officer used physical force to stop the inmate. The Control Booth officer notified the floor officer and sounded the building alarm. A responding officer assisted the attacked officer in subduing the inmate, who continued to be combative. The responding officer used his expandable baton and both officers then placed the inmate in handcuffs. The attacked officer was medically evaluated and taken to an outside hospital for further treatment of injuries to his jaw and lacerations to his fingers.

May 2010

 

May 15 – While staff were passing out the morning breakfast trays, an inmate pushed his tray back out the slot, striking the officer on the foot. The inmate then swung his fist out of the tray slot in an attempt to strike staff. Staff used pepper spray to stop the attack. The Control Booth officer activated his personal alarm; the inmate threw his milk carton at him, striking his leg. Staff responded to the unit and ordered the inmate to submit to handcuffs. 
 
May 15 – Staff approached a solely-occupied cell to escort the inmate. As staff handed the inmate his IEX jumpsuit through the tray slot, without warning the inmate forcefully pulled the suit into the cell causing the officer to jam his thumb on the cell door. The inmate then used profanity to express his joy in injuring the officer. Staff secured the tray slot and left the tier. The officer was medically evaluated. 
April 2010

April 21 – In preparation for an escort, an officer began applying leg restraints to an inmate. The inmate slipped out of his handcuffs, spun around and punched the officer in the eye. The officer used physical force to stop the attack, as responding staff used pepper spray. The inmate was placed on the ground, re-cuffed and a spit net applied. The inmate sustained no injuries. The officer sustained a laceration with active bleeding, pain, swelling and redness to his eyebrow. 

March 2010

 

MARCH 16 --  While escorting an inmate to his yard program, two officers sustained injuries to their legs when the inmate began kicking the officers. Both officers were seen by medical staff and returned to their assigned posts.
 
MARCH 14 --  An officer was assaulted by an inmate as he attempted to remove the inmate’s handcuffs through the food port. The inmate, while restrained, pulled the handcuffs into the cell, injuring the officer's hand. He was medically evaluated and returned to his post. 
 
MARCH 12 --  During the evening cell feeding, an inmate threw an unknown liquid substance through the food port, striking an officer on the side of her body. The officer was medically evaluated for bloodborne pathogen exposure.
January 2010

January 29 – As inmate was in the dental office, the dentist handed the inmate a clipboard with paperwork for him to sign. When the dentist came back to check on the inmate’s progress with the paperwork, the inmate turned and struck the dentist in the groin area. The dentist grabbed the clipboard and the inmate’s hand. The escorting officer grabbed the inmate’s waist restraints and assisted him to his feet. Hewas escorted back to his assigned cell without further incident.

January 3 – Staff responded to the PSU walk-alone yard in response to two inmates gassing each other. Staff ordered both inmates to stop, at which time one of the inmates threw water at the sergeant. Staff used pepper spray to stop the threat. Both inmates were medically evaluated and escorted back to their assigned cells.
 

December 2009

December 27 – In Facility C, Building 2, lower dayroom area, an officer was setting up for medication pass when an inmate, who was in the dayroom to use the phone, came up to the officer and slashed his neck, jaw, and ear. The inmate ran up to the 2nd tier and was chased by the attacked officer and another officer. The inmate complied with orders to get down and was placed in restraints and escorted to a holding cell. The victim officer was escorted to the C facility treatment triage area and was then transported to an outside hospital via Code 3 ambulance. The weapon used was a state used toothbrush with a razor melted into the handle. The officer received 34 stitches to his cheek, 26 stitches to his neck and 8 stitches to his ear.


December 9 – An inmate worker became resistive in a dining room in C Facility. The officer was struck in the face. The officer sustained minor injuries as a result of the attack.


December 5 – A riot involving 10 black inmates occurred. Pepper spray and three 40mm rounds were fired. One inmate-manufactured weapon was recovered. No staff were injured as a result of the riot.


December 5 – As staff escorted an inmate, the inmate began shouting obscenities at the officer. The inmate attempted to pull away from the officer and during the attempt pushed his shoulder into the officer’s shoulder. The inmate then attempted to lunge forward striking the officer. The officer was able to grab the inmate by the arm and force him to the ground. The inmate continued to yell obscenities while on the ground. The escorting officer sustained an abrasion to his knee.
 

November 2009

November 28 - A PSU inmate was being escorted when he spun around and struck the officer in the chin with his shoulder. 

November 27 - In B Facility main yard, 11 inmates began rioting. Staff used 40 mm foam rounds, pepper spray and physical force to stop the melee. There were no injuries to staff. 

November 24 - A PSU inmate was being escorted back to his assigned cell when he turned around as the cell door was closing and spat in the face of one of the escorting officers. The spittle struck the officer on the side of the forehead and dripped into his eye. The cell door was secured and the officer was taken to the Treatment and Triage area for medical evaluation and decontamination. The officer later transported himself to an outside hospital for further evaluation. 

November 23 - An officer was placing handcuffs on an inmate when the inmate grabbed the officer's arm and pulled it in through the food port. The officer suffered minor cuts and scrapes. 

November 19 - An inmate broke out of his handcuffs and punched an officer in the face. Officers were able to subdue the inmate with no further incident. 

November 13 - A SHU inmate broke from an escort, kicking the officer in the groin. The officer was medically evaluated and returned to work.

October 2009

October 29 - While an officer was un-cuffing an inmate from his group return to PSU, the inmate slashed the officer. The officer sustained a cut to the arm.

September 2009

September 10 - During an escort to an exercise yard, an inmate gassed an officer. The officer was medically evaluated and returned to work.
September 6 - An inmate threw his tray through his food port striking an officer. The officer was medically evaluated and no injuries were noted. 
September 6 - Officers were escorting an inmate in A Facility 3 Block when the inmate lunged at an officer striking him in the shoulder. Staff responded and the inmate was placed in a holding cell. Ten minutes later, the inmate was being escorted when he lunged with his head striking the officer in the torso. The inmate was forced down on the floor and held until the medical exam were given. No staff were injured. 
September 6 - Two officers were escorting an inmate, when he lunged toward a sergeant. An officer held the inmates arm and pulled him backward to the ground. The inmate fell backward on top of the officer's leg. The officers assisted the inmate to his feet and continued with the escort. The inmate and the officer were both medically examined and a report was completed. The officer sustained lower back pain and stiffness. The officers then escorted the inmate back to his assigned cell. During the escort, the inmate turned toward the officer and lunged at him. The officer forced the inmate to the ground. The second officer activated his personal alarm. The escort officer struck his left knee on the concrete floor while pulling the inmate away from another officer. The officer sustained an abrasion to the knee. 
September 1 - Two inmates were observed fighting near the handball court on the A Facility Main Yard. Simultaneously, two inmates began running toward the fight when another inmate intervened. The three inmates also began fighting. Responding staff utilized pepper spray to gain compliance with their orders to stop fighting and get down on the ground. One of the inmates from the first fight was laying face down in a pool of blood and the attacking inmate was straddling him and striking him in the back of the head when responding staff arrived. The attacking inmate refused to stop his assault requiring staff to utilize batons to stop his actions. The victim inmate was transported on a gurney and as he was in route to the ER, the inmate attempted to get up causing injury to the officer. The officer sustained an injury to the shoulder and a red swollen foot. The victim inmate received multiple lacerations, bruises and swelling to his face, a cut to the hand and his chain necklace was imbedded in his lower lip. All other inmates received minor injuries. 

August 2009

August 10 - In A-Facility an inmate used urine to gas an officer. The officer was sent to an outside hospital for medical treatment.

February 2009

February 25 – Staff heard an inmate yell for staff to open the cell door of a double occupied cell. Upon arrival, the officer observed blood on one of the inmates and the officer activated the unit alarm. The food port was opened and the bleeding inmate was ordered to submit to handcuffs. As the officer opened the food port and before he could apply gloves, the inmate touched the officer's hand getting blood on the officer. The officer stepped aside to wash the blood off and another officer applied handcuffs and removed the inmate from the cell. The exposed officer was transported to the local hospital for further treatment. One inmate received lacerations, bruises, pain and swelling to his eye and redness and swelling to his shoulder. The other inmate sustained 12 cuts and lacerations to his chest, arm, stomach, back and neck and was transported to a local hospital for a head injury.

February 25 – A Facility EOP staff observed an inmate “square off” with another inmate in the dayroom. The inmate began kicking and punching at the other inmate and at the same time, the victim inmate began punching towards the inmate however no contact was made by either inmate. Floor staff announced the fight and responding staff arrived on the scene. The program activities officer then observed the two inmates engaged in a fight, exchanging blows with clenched fists to the body and face. The officer ordered the combatants to stop fighting and get down. When both inmates refused, the officer fired one 40 mm foam round. The round did not make contact but had the desired effect and the inmates stop fighting. Responding staff restrained the inmates without further incident. No staff were injured as a result.

February 25 – On the main exercise yard in C Facility two inmates were observed fighting. Staff ordered both inmates to get down to which neither complied. Responding staff utilized pepper spray; striking both inmates in the face. The two inmates stopped fighting and laid down on the ground. Both inmates were escorted to the sally port, but refused decontamination showers. No staff were injured as a result of this incident.

February 25 – ISU staff discovered two cellular telephones and 1.1 grams of marijuana during a cell search in C-Facility. The cell was occupied by two inmates with a gang affiliation of the Crips. Both cell phones had photography capabilities. One of the cell phones was internet capable and was activated.

February 19 – An inmate again committed aggravated battery on a peace officer when he spat in the face of an officer. As the officer exited the tier, he passed an inmate in a holding cell. The inmate began yelling profanities and as the officer looked in his direction. The inmate spat on the officer. The spittle struck the officer on his shoulder and face.

February 17 – As the A Facility 5-Block officer was giving an inmate his tray through the food port, the inmate threw the tray back through the food port striking the officer on the arm. The inmate then reached through the food port and grabbed the officer's arm. The unit alarm was sounded and the officer used pepper spray to secure the food port. The inmate immediately obstructed his cell window and barricaded his cell door with his mattress covering the food port and window. The inmate was not responding to staff's orders to remove the mattress and submit to handcuffs. The inmate refused to exit the cell resulting in a cell extraction where an OC x-10 was used to extract the inmate. No staff were injured as a result of this incident.

February 17 – An inmate committed battery on an officer and a licensed clinical social worker. The inmate was being held in a holding cell in the A Facility sally port when he threw his eye glasses at the torso of one officer and as the officer stepped aside the glasses struck another officer on his back. While the social worker was attempting to talk to the inmate he reached out of the tray slot and grabbed the social workers arm. There were no injuries to staff or inmate.

February 7 – Staff detected the odor of marijuana coming from two visitors as they were processing in to visit an inmate. The visitors were interviewed and staff discovered six colored balloons filled with marijuana mixed with instant coffee granules and wrapped in cellophane. During the vehicle search, staff discovered a clear plastic bag containing marijuana on the driver side door panel and a second bag of the same substance. Both visitors were arrested and booked into the county jail.

February 6 – On B Facility, the program officer and floor officer observed an inmate striking another inmate in the face and chest while on the yard. Two inmates ran to the scene and joined in the attack of the victim inmate. The unit alarm was activated and the inmates were ordered to get down. The inmates did not comply and continued striking the victim inmate. The program activities officer fired one 40 mm direct impact round. One of the inmates wrapped his arm around the victim inmate's neck and began choking him. The PAO officer then fired a second 40 mm round, aimed at and striking the assailant in the buttock; the inmate immediately got down on the ground. Two other inmates continued their attack on the victim inmate. The PAO feared for the victim inmate's life due to the continued choking and the victim appearing to lose consciousness and unable to defend himself, attempted to chamber a round in his Mini-14 but the round failed to chamber properly. As the PAO attempted to clear his Mini-14, the control officer responded to the yard observation window and fired one 40 mm round; the round had no success and the inmates continued the attack. The control officer then fired another 40 mm round striking one of the inmates in the leg. All inmates then complied and got down on the ground. The victim inmate sustained abrasions, lacerations, bleeding and swelling to the eye, nose and mouth, swelling on the hand and a swollen area on his head. The victim inmate was subsequently transported to an outside hospital for further medical examination and treatment. Prior to the transport, the victim inmate was found in possession of an inmate-manufactured weapon in his waistband. The weapon was made of what appeared to be a melted plastic material, measuring about 5 inches in length, about 1.5 inches wide, sharpened to a point at one end, and having a handle made of tape on the other end. One of the assailant inmates sustained abrasions to the knuckles and another assailant inmate sustained abrasions to the knees. No staff were injured.

February 1 – An inmate started a fire at the bottom of his cell. Responding officers arrived at the cell and an officer extinguished the fire. The officers then ordered the inmate to submit to handcuffs. After several orders, the inmate finally complied and the officers safety removed him from his cell. Two officers began escorting the inmate to a holding cell. During the escort the inmate began twisting his body back and forth; however control was maintained. When the escort was in front of the holding cell, the inmate suddenly lunged to his right and head butted one of the officers on the side of his face. Staff used physical force to gain control of the inmate who continued to kick the officers. 

January 2009

January 19 - While staff were picking up trays and trash after the morning meal, they approached a single cell and opened the food port. One of the officers asked the inmate if he had any trays or trash to throw out and the inmate picked up his tray and walked towards the cell door. The inmate then threw the tray at the open tray port causing the contents to strike the officer on the thigh and leg. The officer closed the food port and continued picking up trays.

January 17 - Staff were collecting the evening meal food trays when they stopped in front of a single cell and opened the food port to collect the tray. The inmate forced the tray out of the port and struck the officer's hand. The inmate then attempted to grab the officer's arm by reaching through the food port. The officer was able to step away from the food port to avoid the assault and the inmate pulled his arm back into the cell. The officer secured the door on the food port. No staff injuries were reported.

January 16 - The PSU Legal officer was delivering legal supplies to an inmate at his cell. The tray slot was opened and the officer instructed the inmate to sign the receipt. The inmate suddenly grabbed the officer's wrist and tried to pull him towards the cell door. The officer pulled away losing his balance and falling back onto the ground. The officer ordered the inmate to give the manila folder back, but the inmate refused and began yelling profanities at the officer. A supervisor responded and was able to get the folder back from the inmate without further incident. No injuries were noted on the officer.

January 16 - In the C Facility Clinic an inmate sexually battered an LVN. The LVN was bent over a desk writing when the inmate walked by and grabbed her buttocks.

January 13 - Staff escorted an inmate to the A-Facility TTA for examination. The inmate jumped up from the exam table and kicked one of the officers on his thigh. The other escorting officer pushed the inmate away from the first officer and they both forced the inmate to the floor and requested assistance. The unit sergeant responded and placed his hand on the inmate's arm while leg restraints were applied.

January 8 - An inmate head butted a correctional officer in the right facial area. Staff utilized physical force to control the inmate's actions.

January 8 - Two inmates on Facility C stabbed another inmate on the main yard. One 40mm Direct Impact Foam Baton round and pepper spray were utilized to quell the incident. The victim inmate sustained punctures to the chest and right arm. No staff were injured.

December 2008

December 19 – Two officers were removing restraints from an inmate's legs when the inmate started kicking at the officers. The inmate struck one of the officers in his leg. Staff forced the inmate to the ground. While taking the inmate to the ground, the officer struck his knee on the floor. The inmate's legs were controlled while staff reapplied the leg restraints. The RN medically evaluated the officer who sustained a bruise/discolored area to his leg as a result of this incident. The inmate sustained an abrasion/scratch to his chin.

December 12 – An inmate spit on an officer. There were no staff injuries as a result of the incident and no use of force used.

December 11 – While an officer was collecting the evening meal trays when he approached a cell and attempted to collect the tray. The officer opened the tray slot and the inmate placed the food tray on the tray slot and then shoved the tray towards the officer striking his thigh. The officer immediately closed the tray slot and left the area.

December 1 – An officer opened an inmate's food port to place him in handcuffs for medication pass. The officer observed the inmate eating an apple with his left hand as he opened the food port. When the port was completely open, the inmate used his right hand to throw an unknown liquid substance onto the officer's hip. The officer immediately closed the food port and notified the sergeant of the incident.

September 2008

September 10 - While serving the morning meal, an officer opened the food port of a solely occupied cell and began to hand the inmate his tray. The inmate suddenly reached his other hand out of the food port and grabbed the officer's lanyard. The unit alarm was activated and the officer ordered the inmate to stop but the inmate would not comply. The officer utilized his pepper spray and ordered the inmate to stop; to which the inmate complied. The officer was seen and evaluated by onsite medical staff. No injuries were reported.

August 2008

August 26 - While lunches were being passed out in FAB 4, an officer opened the food port to a solely occupied cell. As the officer handed the inmate his lunch, the inmate threw a large amount of an unknown brown liquid substance that smelled like feces at the officer, hitting him in the face, the front of his uniform and the front of both legs. The second officer sustained a small amount of the brown liquid on the back of her uniform as she turned away. The unit alarm was sounded, responding staff arrived and the inmate was ordered to go to the back of his cell and he complied. The officer was sent to an outside medical facility for treatment and evaluation.

August 15 - An officer passing out lunches in a housing unit was opening a food port when an inmate thrusted his arm out and took control of the food port. The inmate then threw the contents of a container with an unknown liquid substance at the officer, hitting him on his leg and upper back. The officer activated his personal alarm. Responding staff arrived and pepper spray was used to stop the attack.

August 14 - An officer observed an inmate bleeding from a self-inflicted wound on his face. The officer along with another officer escorted the inmate to a treatment module to be seen by medical staff. The inmate began to throw his head back and forth splattering blood on both officers. The inmate complied with staff orders to stop. One officer had blood on the arm and jumpsuit, the other officer had blood on both arms, head and jumpsuit. They were seen and evaluated by medical staff and then sent to an outside medical facility.

August 14 - While cell feeding in the Ad Seg unit, an officer was handing an inmate his food tray, when the inmate shoved the tray back at the officer. The inmate continued his forward thrust in order to grab the officer's lanyard. As the officer pulled his lanyard from the inmate's grasp, he activated his personal alarm and warned the inmate to pull his hand back in and to step away from the food port. As the inmate continued to reach for the officer, the officer utilized his pepper spray and gained compliance from the inmate. Responding staff placed handcuffs on the inmate and escorted him to decontamination.

August 14 - An officer was passing out drinks for the evening meal when he approached a solely occupied cell. The officer filled the inmate's container and was giving it back to him when the inmate threw the contents of another container containing an unknown liquid striking the officer on the right side of his neck, chest and shoulder. The officer was evaluated by medical staff and then sent to an outside hospital for further evaluation and treatment.

July 2008

July 23 - An officer was supervising the morning meal, when an inmate turned and punched the officer in the head. Responding staff was able to subdue the inmate. The officer sustained a contusion to the head and responding staff received minor injuries. The officers received medical treatment at a local hospital.

July 10 - An officer opened the food port of a solely occupied cell handing the tray to the inmate. The inmate took the tray and then forcefully shoved the tray back at the officer, striking the officer on the hand and spilling hot cereal on the officer's leg. The inmate continued to try and strike the officer with the tray by holding and swinging the tray. The officer utilized pepper spray to stop the attack. The officer sustained pain on the hand.

June 2008

June 3 - A riot occurred on a main exercise yard involving 11 inmates, four were Del Paso Heights Bloods and seven Oak Park Bloods. Responding staff used 40mm rounds, pepper spray, and grenades to stop the incident. One inmate-manufactured weapon was recovered during the incident. Another weapon was found uncontrolled near the incident. All involved inmates were placed in ASU.

June 3 - An officer was walking past the holding cells in ASU where an inmate was awaiting an escort, when the inmate spat on the officer striking him on the forearm. The inmate continued to keep his face in the cuff port spitting in the direction of the officer. The inmate refused orders to remove his face from the cuff port. The officer utilized pepper spray striking the inmate in the facial area.

May 2008

May 27 - An inmate refused to submit to an unclothed body search after returning late from yard recall. Staff repeated the order. The inmate complied but began throwing his clothing to the ground. The inmate was asked to submit to handcuffs. As custody staff reached for the inmate's arm, the inmate turned aggressively raising his elbow in an attempt to strike an officer. Staff applied downward pressure to the inmate's arm and shoulders forcing him to the ground. The inmate continued to resist by thrashing his body and trying to get up. Staff applied leg restraints, lifted the inmate from the ground and escorted him to a holding cell. One officer sustained a right shoulder and elbow strain and a reddened knee but returned to work.

May 22 - An officer was spit on by a PSU inmate, striking the officer in the neck and arm. No force was used.

May 18 - Officers were assisting medical staff to administer a doctor ordered emergency medication to an inmate in a holding cell. The inmate rushed the cell door as it was being opened and kicked an officer in the stomach. The inmate was forced to the ground but continued to struggle by thrashing his body back and forth. Offices applied leg restraints and medical staff administer the emergency medication.

May 14 - An inmate in a holding cell became agitated and attempted to break his handcuffs off on the corner of the cell table. When the escort officer ordered the inmate to stop his actions, he spit at the officer. The officer utilized pepper spray to stop the inmate's actions. Staff utilized a hand held metal detector during the contamination process and detected metal contraband in the inmate's anal cavity. After the decontamination shower, the inmate voluntarily removed the metal items and admitted to having additional metal contraband in his cell and inside of his state issued blue coat.

May 11 - An inmate threw an unknown liquid at two officers escorting an inmate on a management yard. When the liquid struck one of the officers on his face and upper body, he used his pepper spray on the inmate, who then stopped his assault and retreated to the far side of the yard.

May 10 - While staff conducted a cell search, the inmate complied with orders to exit his cell but then moved back into the cell as he was removing an object from his sock. Staff utilized pepper spray in an attempt to stop the inmate from flushing the object down the toilet. The inmate was able to flush the object and staff restrained the inmate and escorted him to be decontaminated. Staff accessed the sewer clean-out and recovered an inmate-manufactured weapon, a plastic rod measuring six inches long.

May 8 - A PSU inmate had broken out the rear window of his cell and was acting erratically inside his cell. The inmate refused all orders to calm down and submit to handcuffs. Staff standing outside of the broken cell window used pepper spray each time the inmate attempted to climb out of the broken window. The inmate was yelling and thrashing about in the glass on the cell floor and doing somersaults onto the glass. An extraction team was assembled as staff attempted to get the inmate to calm down. Pepper spray had no noticeable effect on the inmate and he continued to wander around the cell, lying in the glass and acting erratically. A gas dispersion was discharged into the cell. As the inmate placed his head in the toilet, the extraction team entered the cell in an attempt to restrain the inmate. The inmate continued to fight officers and was covered in blood and pepper spray. As staff were overwhelmed with pepper spray they were replaced by a second extraction team standing by. After some time, the inmate was restrained and the officers removed him from the cell and escorted him to be decontaminated. During the escort and decontamination the inmate continued to fight the officers. He was resistive throughout the escort and while being medically evaluated. Medication administered several times in an attempt to calm him down had no effect. When the inmate finally calmed down he was transported to an outside hospital.

May 8 - While escorting an inmate to his assigned cell, the cellmate ran out of the cell pushing the officer back. The unit sergeant responded to the upper tier ordering the inmate to get down on the ground. The inmate refused the order. The sergeant grabbed the inmate and turned him around placing him against the wall as staff handcuffed him.

April 2008

April 13 - Two inmates were involved in mutual combat on the basketball court of A Facility Main Yard. Officers ordered the combatants to stop fighting, to which the combatants did not comply. The Officers utilized OC pepper spray. The Sergeant responded and ordered one of the inmates to stop fighting; which he did not comply. The Sergeant utilized his expandable baton and struck one of the inmates. Both inmates stopped fighting and got down in a prone position on the ground. The inmates were decontaminated and medically evaluated. One inmate received abrasions to the shoulder, back and knees. No injuries were sustained by the other inmate.

March 22 - The EOP Floor Officers were escorting an inmate from his assigned cell to the holding cell for medical clearance due to an indecent exposure that had happened earlier. As the escort came down the staircase, the inmate kicked one of the Officers. The Officer then used physical force to place the inmate on the ground. The other Officer helped control the inmate's legs. The Sergeant responded to the incident and placed a spit net on the inmate. Leg restraints were also placed on the inmate. The inmate was medically cleared and re-housed without further incident.

March 20 - Two Mexican National inmates assaulted and stabbed another Mexican National inmate on the B Facility Main Yard. Staff utilized one Exact Impact round, OC pepper spray, OC grenade, and physical force to quell the incident. The victim inmate received three puncture wounds to the abdomen and head trauma. The inmate was transported to UCD Medical Center. The inmate manufactured weapon measured approximately six inches in length and was made of a metal rod sharpened to a point. As a result of this incident all suspected Mexican Nationals and/or associates housed in 6 Block were placed on modified program pending an assessment and observation period.

March 19 - On B Facility Main Yard, a Southern Hispanic was the victim of a battery with a weapon. The severity of the incident posed a significant risk to the safety and security of the institution and delayed the normal operations of the facility. As a result of this incident all suspected Southern Hispanic inmates and/or associates housed in 3 Block were placed on modified program pending an assessment and observation period.

March 17 - An inmate covered his cell windows with paper and was unresponsive. Staff formed an emergency extraction team and entered the cell. The inmate suddenly became combative and assaulted the Shield Officer and second Officer that entered the cell. Staff were taken to an area hospital. One of the Officers sustained a split kneecap and needed emergency surgery. The other Officer received numerous cuts, abrasions and bruising to his face. The inmate was treated for minor injuries and returned to his cell.

March 10 - In a dayroom an inmate committed battery on another inmate, resulting in serious bodily injury. The victim inmate was transported to UC Davis for further medical treatment. No force was used and no injuries were incurred to staff.

March 10 - B Facility 5-block Program Activities Officer observed two inmates engaged in mutual combat. The Officer ordered the inmates to stop and get down but the inmates continued to fight. Responding staff ordered the inmate to stop fighting then administered OC pepper spray. The inmates separated and assumed a prone position on the ground. Both inmates were decontaminated and medically evaluated.

March 7 - Two Southern Hispanic inmates were being escorted from B Facility to the sally port while EOP yard was out. The Officers doing the escort ordered four EOP inmates to get off the track for the escort and the EOP inmates ignored the order and kept walking towards the escort. The escorting Officers ordered the EOP inmates two more times to move off the track for the escort before they complied. As the escort Officers passed the EOP inmates, the two Southern inmates ran towards the EOP inmates and started fighting with them. The escorting Officers used OC pepper spray and then their Baton to get the inmates to stop fighting. There were no staff injuries and only minor injuries to the inmates.

March 7 - While the EOP Floor Officer was escorting a LVN, an inmate requested medication from the LVN. The Officer signaled the Control Booth Officer to open the cell. The inmate stepped out of his cell and took a bladed aggressive stance and became verbally threatening. The Officer drew his MK-9 and ordered the inmate to return to his cell. The inmate ignored the verbal order and attempted to strike the Officer. The Officer was able to move out of the way but the inmate was able to strike the Officer. The Officer then sprayed the inmate with his MK-9 striking in the facial area. One of the Officers then grabbed the inmate by his shoulders and forced him to the ground. The inmate was handcuffed and medically evaluated. No staff injuries were reported.

March 4 - While on the lower handball court, an inmate attacked another inmate with a weapon made from a rusty nail. Staff used OC pepper spray to quell the incident.

February 2008

February 25 - While on the A Facility Small Side Yard, an inmate swung at an Officer. The Officer turned to the left causing the inmate to miss his face and striking him in the back shoulder area. The inmate took another swing, which struck the Officer on the back. The inmate continued his attack as he was being sprayed with OC pepper spray. The inmate and the Officer fell to the ground. The inmate refused to comply with staff orders to stay down on the ground. As the inmate was attempting to stand up, staff struck him once in the shoulder with a forward strike from an expandable baton. The inmate remained on the ground and submitted to restraints without further incident. The Officer sustained no visible injuries, but complained of some pain. The inmate sustained a wound to the facial area and swelling to the shoulder.

February 24 - Staff observed an inmate walking down the tier and being pushed inside a cell. The inmate backed out of the cell attempting to fend off an attack by another inmate. After exiting the cell, another inmate joined in on the assault. The unit alarm was sounded and the staff processing inmates to the yard immediately responded to the attack. As staff entered the unit and gave orders to stop the attack, the two inmates complied. Responding staff observed one of the inmates dropping an inmate manufactured weapon from the second tier onto the first tier. The weapon was fashioned from round metal stock, possibly oven rack material, measuring approximately 8 inches long by 1/4 inch in diameter. No force was used by staff. The inmate victim was transported to UC Davis Hospital.

February 14 - An inmate approached another inmate in a joking manner and was observed placing his arm around the victim's shoulders before making a slicing motion to the victim's neck. The inmate fell to the ground with blood spewing from his neck. The assailant dropped the weapon, stepped away, and got down on the ground. The alarm was sounded and the assailant restrained as responding staff immediately started life saving measures on the victim. The victim was transported to Folsom Mercy Hospital where he was pronounced dead.

February 13 - Two officers ordered an inmate porter not performing his duties to lockup. The inmate ignored the order and punched one of the officers in the face as she attempted to handcuff him, knocking her unconscious. The inmate then rushed the other officer and began to strike him repeated in the head. The officer was able to grab the inmate and force him to the ground. The inmate continued to strike the officer until responding staff arrived and gained control of the inmate. The inmate was medically evaluated and sustained abrasions and pain to his knee and ankle. The inmate was placed into ASU for Battery on a Peace Officer. The female officer was taken to the hospital for evaluation. The other officer suffered a red, swollen and bruised eye.

February 11 - A racial melee occurred on the main exercise yard involving nine Hispanic and Black inmates. One 40mm Direct Impact Round and OC pepper spray was used to quell the incident.

February 11 - Two Southern Hispanic inmates began to assault and stab a Black inmate with a 6 inch metal rod. Two 40mm Direct Impact Rounds, two Mini-14 warning shots, and side handle baton was used to quell the incident. The victim received numerous superficial puncture wounds and was treated on site.

February 7 - A Black inmate struck an Asian inmate on the main exercise yard. Immediately 55 Crip gang members and Asian inmates ran to engage in the melee. Code Three Response was immediately called. Two 40 mm rounds were fired and OC grenades were used to stop the incident. Two Black inmates and one Asian inmate were transported to an outside hospital for blunt force trauma to the head.

February 6 - Four inmates in the gym/dorm were involved in a fight. Staff used OC pepper spray and a baton to gain control of the incident. One of the inmates was transferred to an outside hospital. A search of the dorm resulted in four inmates possessing weapons.

February 5 - Two white inmates assaulted and stabbed a third white inmate while on a main yard. The victim was transported to an outside hospital Code 3 for multiple stab wounds.

February 4 - An inmate being served a CDCR 115 became disruptive and the floor officer ordered the inmate to return to his cell. The inmate moved toward the officer, pushed him and then struck him in the face with his fist. The officer responded by grabbing the inmate's arm with both hands trying to force him to the ground. The inmate continued to strike the officer as staff activated the unit alarm and attempted to pull the inmate away from the officer. Staff was not able to stop the inmate's assault. Additional responding staff arrived and was able to pull the inmate to the ground and restrain him. The inmate sustained abrasions to the ankles, bruises to the mouth, wrist, and shoulder blade. The officer sustained abrasions on his nose, side of his jaw, elbow, and his head. The officer was transported to an outside hospital for further evaluation.

January 2008

January 23 - An inmate was found dead in his cell by a Doctor conducting rounds who reported that the inmate was unresponsive. The inmate was found with a plastic garbage bag tied around his neck. The inmate and his cellmate were both members of the "Blood" street gang but were from different regions of California. Both inmates had been placed in Administrative Segregation after committing a stabbing assault together. The cellmate is suspected of murder.

January 18 - An officer was head-butted in the face by a PSU inmate who was being escorted to group. Staff were able to restrain the inmate. The officer sustained a reddened area to the right side of his face.

January 16 - A cell extraction was required for an inmate who refused to be transported to another institution. During the extraction, the inmate struck an officer in the head.
