March 15 – In Bravo Facility Building 1 an officer was rehousing an orientation inmate in another cell. Once the cell door was opened the inmate that was already housed in the cell rushed out and grabbed the officer. The officer was able to pull away and use pepper spray to stop the inmate. The inmate was then placed in mechanical restraints with no further incident. The officer was taken to medical for evaluation and released.
February 2011
February 28 – Ad Seg staff escorted an inmate to be rehoused. Once officers arrived at his cell and removed his restraints the inmate turned and grabbed one of the escorting officer’s hand and kicked the officer in the leg. The officers grabbed the inmate and put him in a prone position and placed him in restraints without further incident. The officers were evaluated by medical and remained on duty.
July 2010
July 4 – On B yard, a black inmate, disgruntled over a cell move, threw an unknown substance at an officer. The inmate then attempted to fist fight and stab the officer.
May 2010
May 15 – Staff responded to a “man down” call coming from a double occupied cell. As staff arrived, they observed an inmate sitting on the lower bunk bleeding from the side of his head and above his eye. The housing unit alarm was activated and responding staff arrived and opened the cell door. Both inmates were removed from the cell, placed in handcuffs and escorted to program services. Medical evaluations determined the inmate sustained slashing-type wounds to his head and eye. Subsequently, the inmate was transported to an outside hospital for treatment and returned to the prison. The cell was processed as a crime scene, however the weapon was not located. Both inmates were medically cleared and rehoused in Ad Seg.

December 2009
December 2 – Staff observed two inmates committing battery on another inmate on the recreation yard. Both inmates were striking the inmate on the face and torso. The yard was ordered down and all inmates complied except the assailants who continued their attack. Numerous orders to get down were ignored. Staff used 40mm to gain compliance. Responding staff placed the inmates in restraints and escorted them for medical evaluation. The attacked inmate sustained four puncture wounds to his back and one of the assailant inmates sustained one puncture wound to his elbow. During a search of the incident area, staff recovered one inmate-manufactured stabbing weapon.
November 2009
November 1 - An officer observed a Northern Hispanic inmate hand an unknown object to another Northern Hispanic inmate. As the officer approached the inmate, he placed a yellow bindle into his mouth and walked away. The officer activated his personal alarm and ordered the inmate to get down. The inmate refused to comply with staff orders. As the officer pursued the inmate, a second inmate pushed the officer, knocking him off balance. The item was retrieved and identified as a kite. No injuries were reported.

September 10 - Two inmates refused to return to their cells after recreation yard recall. Officers gave verbal orders to the inmates to return to their assigned cell with negative results. Both inmates approached staff in an aggressive manner and were ordered to get down, with negative results. Pepper spray was used on both inmates. One inmate assumed a prone position on the ground, the second inmate recovered from the pepper spray and attacked the officer striking him in the head. The first inmate got up and attacked a second officer striking him in the face. Five exact impact rounds were fired to stop the assault. Both officers were taken to CTC for medical evaluation. Both officer sustained injuries to the head and face. The officers were subsequently transported to an outside hospital for additional medical evaluation and treatment.
September 10 - Two inmates were given several orders to report to their cell and lock up. The inmates proceeded down the stairs towards the officers. One of the inmates continued to ignore verbal orders to return to his assigned cell. The inmate then lunged towards one officer necessitating the use of pepper spray. The inmate remained standing and refused to get down. The second inmate began to approach the officers. The inmate was ordered to get down with negative results and was sprayed in the face with pepper spray; then the inmate complied with orders. The housing unit alarm was activated and simultaneously the first inmate began repeatedly striking one of the officers in the face. The second inmate then got up from a prone position and ran towards the other officer who slipped in pepper spray and fell to his knees. The inmate began striking the officer in the face necessitating the Control Booth officer to utilize the 40mm launcher. Responding staff arrived and both inmates resisted, necessitating physical force to place them in restraints. The two inmates were medically cleared and re-housed in Ad Seg. Both officers were transported to an outside hospital for medical treatment.
September 4 - An inmate was walking towards his cell when he abruptly turned around and punched an officer in the head. The inmate continued to swing at the officer with his fists. The housing unit alarm was sounded. The officer and a Sergeant utilized physical force to take the inmate to the ground. The inmate continued to resist by swinging and refusing orders to be placed in handcuffs. Responding staff arrived and the inmate continued to resist, necessitating the use of pepper spray to gain compliance. Staff received minor injuries and blood exposure as a result of the incident. Staff were medically evaluated and exposure control measures were initiated.
