April 2011
April 30 – A psychologist was conducting an individual counseling session when the inmate threatened to kill an officer during his next escort.
April 29 – An inmate began rubbing his genital area through his clothing while watching a female officer. The officer ordered the inmate to stop with negative results. The officer left the area and reported this incident to her sergeant.
April 28 – Two Southern Hispanic inmates attacked another Southern Hispanic inmate during the morning yard on Facility B. An officer used one grenade to stop the attack, which had positive results. Once the scene was secured all three inmates were removed from the area anddecontaminated.
Also on April 28 – An officer ordered the inmate to stop kicking his cell door with negative results as the inmate continued to kick the door. Fearing for the structural integrity of the door, the officer used pepper spray striking the inmate in the back, head and face. The inmate then ran to the back of the cell wrapping his shirt around his face to diminish the effects of the pepper spray. The inmate appeared to be preparing for a possible extraction, so the officer used more pepper spray striking the inmate in the face. The pepper spray had the desired effect and the inmate complied with orders to submit to an unclothed body search and restraints.
Also on April 28 – During medication pass an inmate threatened to spit in a Licensed Psychiatric Technician’s face.
April 25 – An inmate was ordered to stop kicking his cell door or pepper spray would be used. The inmate then grabbed a blanket and ran to the cell front to cover the door. Pepper spray was used to stop the inmate’s actions and gain compliance.
April 23 – While a psychologist was conducting an individual counseling session, she observed the inmate lean back in the holding cell, elevate his knees exposing his genitals to her. The psychologist immediately exited the room.
Also on April 23 – While at a cell door attempting to conduct an unclothed body search, the inmate suddenly spun around and turned off the cell light then quickly moved to the rear of the cell with his back facing the officers. An officer used pepper spray while at the same time the other officer ordered the inmate to stop. The inmate refused to comply and continued to look for something. The officer applied a second application of pepper spray with the same results. A third application of pepper spray was used, this time with positive results as the inmate submitted to handcuffs.
Also on April 23 – While conducting a search of a solely occupied cell, a sergeant discovered an inmate-manufactured weapon measuring 6 ¼ inches long and ¼ inch wide. The handle was made from tightly rolled paper and the metal tip appeared to be made from a razor blade.
April 22 – An inmate battered an officer by kicking him in the groin while being treated in the psychiatric technician’s office. The officer used physical force and placed the inmate on the ground.
April 21 – An inmate shot a spear out of his cell door striking an inmate en route to the shower. An officer secured the spear and searched the cell. The officer located and secured two additional spears that the inmate attempted to destroy prior to submitting to handcuffs.
April 20 – An officer discovered three inmate-manufactured weapons within the personal property of an inmate. The inmate was charged with possession/manufacture of a deadly weapon.
April 19 – Staff were conducting trash and tray pickup when an inmate threw the tray and food items at the officers.
Also on April 19 – An inmate refused to exit his cell for a cell search. The inmate then attempted to prepare for a Controlled Use of Force so pepper spray was used to stop the inmate’s actions and gain compliance.
April 18 – Two Southern Hispanic inmates were attacked by three Northern inmates on a yard. Staff responded and used pepper spray and baton strikes to stop the riot. The involved inmates were secured and escorted to holding cells.
April 16 – An inmate was yelling and kicking on his cell door. Officers ordered the inmate to stop. The inmate told the officer to open the cell door and he would stab them. The inmate then grabbed a blanket and began to cover himself. An officer used pepper spray to stop the inmate’s actions. The inmate then submitted to handcuffs and removed from the cell. Following a decontamination shower and secured in a holding cell the inmate threatened to rape the officer and his wife and molest his children.
April 15 – An inmate was kicking his cell door and barricading himself in preparation of a cell extraction. Pepper spray was used to stop the inmate’s actions.
Also on April 15 – While a Licensed Clinical Social Worker was conducting a one-on-one in the PSU Treatment Center, she observed the inmate rubbing his penis over his Indecent Exposure Jumpsuit. The LCSW left the room and informed staff of the inmate’s actions. The inmate had nine previous indecent exposure violations. Two officers then began to escort the inmate back to his cell when the inmate attempted to kick one of the officers. The officers used physical force to stop the inmate and place him on the ground. Another officer responded and applied leg irons.
Also on April 15 – An inmate refused to exit the shower and stated he wanted to speak to a sergeant. A sergeant responded and ordered the inmate to submit to handcuffs and exit the shower, to which the inmate refused. The inmate then began to place a towel over his head and refused an officer’s order to stop his actions. The officer used pepper spray to stop the inmate and gain compliance.
April 11 – An inmate began banging his head against the door and concrete wall of his cell. An officer ordered the inmate to stop, warning that pepper spray would be used if the inmate continued. Fearing the inmate would sustain serious injury if allowed to continue, the officer used pepper spray striking the inmate in the torso and face. The inmate stopped his actions and was removed from the cell for decontamination and medical evaluation.
April 8 – An inmate was observed kicking and striking his cell door with his fists. An officer ordered the inmate to stop. The officer then opened the cuff port as the inmate attempted to block the opening with a blanket while continuing to kick the door. The officer used pepper spray to stop the inmate.
April 7 – An officer discovered an outgoing letter authored by an inmate to an outside party which gave instructions to route money to another inmate for the purpose of avoiding court ordered restitution.
Also on April 7 – An inmate was placed into Contraband Surveillance Watch for suspicion of possessing inmate generated gang notes secreted in his rectum. The inmate lunged forward and head butted the sergeant. The sergeant used physical force and placed the inmate on the floor. As the sergeant and officer held the inmate on the floor an alarm was sounded. As responding staff arrived, they secured the inmate into leg restraints while CSW Restraint equipment was placed on the inmate. The sergeant sustained an abrasion to his forearm along with bruising to his arm and shin.
April 6 – While in a holding cell, an inmate threatened to punch an officer in the mouth.
April 5 – An inmate refused to comply with a cell search after he was suspected of hoarding medication. The inmate began to prepare for a possible extraction by barricading his cell. Pepper spray was used to stop the inmate’s actions and gain compliance with staff orders.
April 4 – A psychologist was conducting a mental health interview with an inmate when the inmate threw a plastic bag containing two remote controls on the floor. An officer responded to the office and was informed of the incident. The officer told the inmate the interview was over and to stand up. As the inmate stood up he knocked the phone off the desk and onto the floor. The officer placed the inmate against the wall and the inmate then attempted to strike the officer. The officer used physical strength and body weight to force the inmate to the ground. Another officer responded and assisted in placing the inmate in handcuffs.

March 31 – A pod search was being conducted when staff observed two inmates ingesting and destroying inmate-manufactured notes. The two inmates refused numerous orders from staff to stop. As they continued to tear up the notes and ingest them, they placed several torn up strips of paper in the toilet. One of the inmates then placed a sheet in front of the other inmate, obstructing staff’s view of the second inmate. An officer and Lieutenant gave an order to take the sheet down with negative results. The officer used pepper spray striking the inmate in the hip and back as the inmate continued to destroy the notes. The other inmate was still obscured from staff. A second burst of pepper spray was used, striking the inmate in the head. This had a positive effect as the inmate removed the sheet and both inmates complied with staff orders and submitted to handcuffs without further incident.
March 28 – An inmate was given a direct order to stop kicking a cell door, and the inmate complied. The inmate was then separated from his cellmate and taken to a holding cell for a medical evaluation. The inmate immediately began to kick the holding cell door. The inmate was again given a direct order to stop, but the inmate continued to kick and strike his head against the door presenting an immediate threat to the safety of himself. The inmate refused orders to stop so pepper spray was used to stop his violent actions. The inmate was restrained and escorted out of the cell for decontamination and a medical evaluation.
March 25 – Two officers were escorting an inmate to his cell when the inmate became agitated. The inmate quickly turned toward one of the officers and lunged at him forcefully striking the officer using his head and shoulder. The officer reacted by raising his forearm up and blocking the inmate’s strike. The two officers responded to the attack and forced the inmate down to the floor and held him as the inmate resisted them by trying to turn from side to side. Responding staff arrived and placed the inmate into a Stokes Liter and then up on a gurney as a precautionary measure because the inmate was highly volatile. One of the officers sustained pain to the forearm and the other officer sustained bruising to his knee. Both officers returned to work.
March 18 – An inmate made threatening statements to an officer. The inmate was charged with Threats of Force and Violence Against a Public Official.
March 12 – A Floor Officer was in the process of conducting a standing count, when she observed an inmate in a sole occupant cell laying on his bunk with his boxer shorts pulled down to his knees, masturbating as he made eye contact with her.
Also on March 12 – An inmate refused to exit his cell to allow staff to search the cell. A Sergeant was notified and ordered the inmate to exit the cell. The inmate refused to comply. An extraction team was assembled. The inmate informed the Sergeant that there would be a fight, as he applied a towel around his face in preparation of the extraction. The Sergeant seeing the inmate attempting to prepare for the extraction used immediate force to stop the inmate by opening the food port and placing one grenade in the cell. The inmate recovered the grenade and placed it in the toilet. The inmate refused to remove the towel, so the Sergeant used a second grenade in the cell. The inmate refused orders to stop, so the Sergeant deployed another grenade in the cell. The inmate was unable to recover the grenade before it deployed. This had the desired effect and after several minutes the inmate complied with staff. This is the same inmate who was involved in the March 11 incident of threatening an officer.
March 11 – An inmate threatened an officer during count. The inmate threatened to “get ready for the next time you open the expletive port. I am going to expletive you up bad, just wait and see.”
March 3 – During medical pass an inmate threatened to gas an unknown substance on an officer. The officer deemed the inmate’s threat to be viable and the inmate was subsequently removed from his assigned cell with no further incident.
March 2 – An officer ordered an inmate to submit to restraints for the purpose of a cell search and refused. A Sergeant responded and ordered the inmate to submit to handcuffing and he again refused. A Captain responded and authorized the use of Controlled Force to gain the inmate’s compliance. A Sergeant deployed one grenade through the cuffing port. After several more orders were given to the inmate he submitted to an unclothed body search. Two officers responded and placed the inmate in handcuffs and leg irons. The inmate was escorted to the unit rotunda and examined. No staff were injured.
March 1 – During a search of Facility B Unit 5 an officer discovered a state-issued razor blade hidden in a Bible in an inmate’s locker. The blade was approximately 1.5 inches long by .25 inches wide.
Also on March 1 – A Sergeant was searching a cell and discovered an inmate-manufactured deadly weapon. The Sergeant secured the weapon and processed it into evidence.
February 2011
February 26 – An inmate refused to comply with orders to submit to handcuffs so he could be moved to another cell. The extraction process was initiated and clinical intervention attempted. An officer used pepper spray to stop the inmate from covering up his cell door in preparation for the extraction. This force was met with positive results in that the inmate ceased his actions. The inmate then continued to refuse orders to submit to handcuffs. Additional officers responded to the cell front as the inmate again attempted to cover his cell door. As the officer was attempting to apply a second application of pepper spray, the inmate grabbed the MK9 and began to pull it into the cell through the open cuff port. The officer maintained his hold on the pepper spray but during the struggle for control, the MK9 broke. The canister portion ended up in the possession of the inmate and the handle portion remained with the officer. When the canister broke the OC sprayed out and struck two officers in the torso, neck and face. The inmate backed away and after the cuff port was closed, he became complaint and submitted to an unclothed body search and handcuffs.
February 12 – As an inmate was kicking a cell door, an officer ordered the inmate to stop. The inmate refused and continued kicking the door. Fearing for the integrity of the door, the officer used pepper spray, striking the inmate in the chest and stomach area. The inmate then grabbed personal clothing and other material and attempted to barricade the door. The officer used pepper spray to stop the inmate’s actions.
February 2 – An inmate was kicking his assigned cell door in the CTC. Two officers ordered the inmate to stop, with negative results. One of the officers told the inmate that chemical agents would be used if he did not comply. The inmate continued to refuse orders. The officer then utilized pepper spray, striking the inmate in the chest. The pepper spray had the desired effect and the inmate stopped kicking the door.
Also on February 2 – An inmate intentionally exposed his penis to a registered dental assistant. The inmate tore a hole in the crotch of his Inmate Exposure Suit, and had his penis protruding out of the hole. The inmate was immediately removed from the dental office and returned to his assigned cell.
January 2011
January 28 – An inmate attempted to strike an officer in the face. Physical force was used to stop the inmate’s actions. The inmate was restrained and secured in handcuffs. An officer sustained pain to the knee.
January 24 – An inmate was in the rotunda being escorted out of the unit when he became resistive towards escorting staff. The inmate began twisting his body in an attempt to break free of the escort. Staff used physical force to place the inmate in a prone position. A sergeant sustained a scratch on the forearm and pain in the right thumb.
Also on January 24 – Officers were processing inmates for the morning group, when an inmate made the following threatening statement to an officer, “I’ll stab an officer, I’m not afraid of no officer.”
Also on January 24 – An inmate became disruptive and began kicking his cell door. Staff used pepper spray to stop the inmate’s actions, with negative results. The inmate then battered staff and a second burst of pepper spray was required to stop the inmate.
January 18 – Officers were at the front of a cell to conduct a search, when the inmates refused to submit to handcuffs. One of the inmates threw an ‘ear bud’ with the wires balled up, through the open cuff port striking an officer in the chest. The inmate then began wrapping his hand with a wet towel and threatened to kill anyone who entered the cell. Another officer ordered the second inmate who was at the rear of the cell not to attempt to cover himself with the blanket he was holding. The second inmate ignored staff orders so the officer used pepper spray striking the blanket. The other inmate charged the open cuff port and the officer discharged another burst of pepper spray striking the inmate in the face. The inmate then grabbed the television in the cell raising it above his head. The officer ordered the inmate to stop with negative results. The officer then used pepper spray striking the inmate in the face. The inmate threw the television against the base of the cell door, breaking the television into countless pieces. The inmates then submitted to handcuffs. As the inmates were escorted from the cell, one of the inmates began threatening staff saying when he gets a chance he will kill one of the staff members. No staff were injured as a result of this incident.
January 15 – An inmate committed an act of sexual misconduct by indecently exposing himself while masturbating to an officer as she was conducting count.
Also on January 15 – As officers were distributing meals to the inmate population, they approached a cell and observed an inmate in a seated position on the floor, leaning against the lower bunk. The inmate was non-responsive at the time. An officer instructed the other occupant of the cell to turn on the cell light. The inmate turned the light on, revealing massive injuries to the inmate. The officer instructed the Control Officer to activate the unit alarm and request assistance. Both officers placed the inmate in handcuffs and escorted him to a holding cell. Responding staff entered the cell and rendered medical aid to the inmate, who had massive trauma to his head and torso. A Code 3 ambulance was requested and the inmate was taken to an outside hospital.
January 11 – Two inmates rushed officers while being released into the exercise yard. Staff in the immediate area responded and stopped the attack using physical force and batons. Two inmate-manufactured weapons were discovered. Three officers were taken to a local hospital for treatment of lacerations and puncture wounds.
Also on January 11 – While an officer was providing coverage of staff members serving the morning meal, the officer noticed a cell light on and the window covering removed. The officer then observed the inmate standing on his toilet with his boxer shorts pulled down to his knees, masturbating while staring at her. As floor staff approached the inmate’s cell, he stepped off the toilet and pulled his boxer shorts back up.
Also on January 11 – During a cell search two inmate-manufactured weapons were discovered. Both inmates were removed from the cell and placed into holding cells then rehoused in contraband Watch Cells. One weapon was approximately a one-eighth inch round piece of metal four inches long and the second weapon was a flat piece of metal approximately one and a half inches wide and seven inches in length, with one end tapered to a sharpened end.
January 8 – An inmate committed an act of sexual misconduct by exposing himself while masturbating to an officer as she was conducting count.
January 6 – An officer was providing coverage from the Building Control Booth for staff to conduct the evening medication pass. The officer observed that a yellow window covering on a solely occupied cell had slipped down, exposing approximately 11 inches of the window. The officer observed the inmate positioning himself in a manner to be seen through the window. The inmate was masturbating and exposing his penis. The officer notified floor staff, who responded to the cell and covered the rest of the window.
Also on January 6 – An inmate refused to take his court ordered Keyhea medication. An extraction team was formed and chemical agents were used to remove the inmate from the cell.
December 2010
December 30 – An inmate became agitated and began kicking the cell door. Staff gave the inmate multiple orders to stop with negative results. Fearing the cell door’s integrity may be compromised; staff used pepper spray to gain the inmate’s cooperation to stop kicking the door.
October 2010
October 25 – During the evening med pass, an inmate exited his cell and battered another inmate. Staff used an Exact Impact round to stop the assault.
October 19 – An inmate battered an officer by lunging towards the officer striking him in the forearm. The officer stopped the inmate’s actions by using physical force. The officer sustained injury to his forearm, received first aid and continued his shift.
October 14 – A LPT was conducting group in the PSU Treatment Center when she observed an inmate with his right hand on his crotch area touching himself. The LPT told the inmate his behavior was not acceptable. The inmate then began arguing when the LPT had him removed from group.
Also on October 14 – An inmate head butted an officer while he was being taken out of a transportation van for a court appearance.
October 13 – An inmate refused to comply with verbal and physical direction of two escorting officers by attempting to break free from the officer’s physical control. The officers attempted to counsel the inmate about his resistance and reiterated verbal directives when the inmate again abruptly resisted staff. The officers placed the inmate in a prone position and was held there until responding officers arrived and secured the inmate’s legs with leg irons and placed him in a Stokes Liter. Responding staff escorted the inmate to the CTC. One officer sustained pain to the arm.
October 12 – An inmate made a verbal threat to batter an officer during cell door medication pass.
October 11 – An inmate who was in an Indecent Exposure jumpsuit and handcuffed behind his back per IDTT plan was secured in a holding cell, reviewing his medical file and being supervised by an officer technician, when the inmate intentionally used a table within the cell to rub his genitals on until he appeared to orgasm. The file review was terminated and the inmate was escorted back to his assigned cell without further incident.
Also on October 11 – An inmate refused his court ordered medication resulting in a controlled Use-of-Force extraction.
October 10 – An inmate exposed his penis to an officer as she was conducting count. The inmate was placed on CTQ status pending bed availability in Ad Seg. The inmate has received six previous Rules Violation Reports for indecent exposure.
October 8 – A Psychiatric Technician was conducting an anger management session and observed an inmate masturbating. The P/T ordered the inmate to stop but the inmate ignored the orders and continued masturbating. The P/T exited the treatment room and informed custody staff. The inmate was removed from class and returned to his assigned cell.
Also on October 8 – An inmate began kicking his cell door with such force that it was separating from the frame, jeopardizing the integrity of the door. An officer and sergeant gave multiple verbal orders to stop with negative results. The officer then used pepper spray, which had the desired effect as the inmate stopped his actions. The inmate was removed from the cell and decontaminated.
October 7 – As an officer was searching an inmate’s incoming property from another institution, three deadly weapons were discovered. The officer found one razor blade in a book, another razor blade in miscellaneous papers and an inmate-manufactured stabbing weapon, sharpened to a point on both ends, measuring 2 ¾ inches in length and 1/8 inch in diameter with one end tightly wrapped with paper forming a handle with “CO” written in ink. The inmate was rehoused in Ad Seg.
Also on October 7 – An inmate attempted to break free from escorting officers, resulting in physical force to stop the inmate’s actions.
October 6 – An inmate became verbally threatening and resistive to an escort. Officers used physical force to stop the inmate’s actions. One officer sustained a fractured rib and was sent to an outside hospital for treatment. The other officer sustained an abrasion to the elbow.
Also on October 6 – An inmate refused numerous verbal orders to kneel down for the placement of leg irons. The inmate suddenly began twisting and turning, resisting staff. An officer used physical strength to stop the inmate as another officer applied leg irons. The inmate was rehoused. An officer sustained pain to the hip and knee.
October 2 – An inmate refused to return to his assigned cell and was then placed in a holding cell pending his voluntary return to his cell. During an escort to use the restroom the inmate resisted the escorting officers who were attempting to return him to the holding cell. Physical force was used to stop the inmate’s actions and the inmate was returned to the holding cell. After a medical evaluation was completed, the inmate was being escorted back to his assigned cell and became resistant to staff efforts to complete the escort. Physical strength and holds were used to stop the inmate’s resistive actions. One officer sustained redness and swelling to the elbow.
September 2010
September 2 – An inmate exited the shower and confronted an officer with clinched fists demanding his property. The officer drew his baton and ordered the inmate to get down because of the perceived physical threat. Two other officers pulled their pepper spray out and demanded the inmate to get down; which he refused. The inmate moved toward one of the officers and began verbally threatening to attack. The officer dispensed the pepper spray as the inmate charged towards the officer swing both fists hitting the officer’s shoulder. The officers used pepper spray to stop any further attacks. The inmate complied with staff orders to get into a prone position. The inmate was placed into restraints and escorted out to be decontaminated. One officer sustained an abrasion to the arm.
August 2010
August 23 – An inmate tried to pull his handcuffs into the cell. The officer utilized physical force to hold the cuffs. Responding staff arrived and assisted in un-handcuffing the inmate. The officer sustained an abrasion and redness to the forearm.
As on August 23 – An inmate assaulted an officer when the inmate pulled away from an escort. Responding staff placed the inmate in a Stokes litter and then gurney. The officer sustained pain to his calf and lower back.
As on August 23 – An inmate lunged at an officer, striking in the chest area. The strike pushed the officer into a nearby wall. Responding staff placed the inmate in a Stokes litter and then a gurney. The officer sustained pain in the elbow and shoulder area.
August 18 – During an escort, an inmate threatened to gas or spear the first staff member that came in front of his cell. The inmate eventually submitted to restraints and was escorted without further incident.
August 17 – An inmate was placed on contraband surveillance watch due to a search of his property indicated he was in possession of a weapon hidden in his body. Once the inmate defecated, one plastic wrapped bundle contained an inmate-manufactured weapon was discovered. The weapon was made from rolled metal, measuring four inches in length and one-half inch in diameter tapered to a point at one end.
August 12 – During an escort, an inmate began to resist staff and rammed his head twice into an office window. Staff used physical force to prevent the inmate from hurting himself and to maintain control.
July 2010
July 20 – As an officer was escorting an inmate, the inmate slipped his right hand out of his restraints and swung his fist at the officer, striking him in the chest. The officer used physical force to stop the inmate’s actions. Responding staff used physical strength and holds to force the inmate to the ground.
July 11 – Officers were picking up the breakfast trays when they opened the port to a cell. The inmate threw an unknown liquid substance on one of the officers. The officer secured the port and both officers left the pod. A sergeant responded to the cell front and instructed the inmate to submit to handcuffs and exit the cell. The inmate refused so an extraction team was organized. During this time the inmate attempted to cover his cell front with a bed sheet. As the sergeant pulled the sheet down, the inmate attempted to throw an unknown liquid on him. The sergeant used pepper spray to stop the attack. The inmate was placed in handcuffs and removed from the cell. The officer was transported to an outside hospital for treatment.
July 1 – Distributing the evening meal, an officer opened a service port to a cell. The inmate immediately threw an unknown liquid substance at the officer, striking her on the side and back. The officer closed the port and observed the inmate moving toward the cell door with a blanket in an effort to barricade the door. The officer used pepper spray to stop the inmate, with negative results. Twenty minutes later the inmate voluntarily submitted to handcuffs and was escorted from the cell. The inmate was rehoused in Ad Seg. The officer was given medical attention.
June 2010
June 29 – An inmate was refusing an order to move to another cell. While an extraction team was being assembled, the inmate attempted to strangle himself with a bath towel. Staff used pepper spray to stop the inmate.
June 27 – An inmate attacked an officer in the rotunda. The inmate used an inmate-manufactured weapon sharpened to a point, approximately four inches in length, in an attempt to commit homicide on a peace officer. Physical force was used by the officer and responding staff to stop the attack. A follow-up search of the inmate’s cell revealed another inmate-manufactured weapon, made of metal stock approximately 1.5 inches in length and sharpened to a point.
June 17 – A Security Squad officer found an inmate-manufactured weapon in a cell occupied by two inmates. Both inmates were placed on contraband surveillance watch.
Also on June 17 – An officer observed two inmates fighting in a day room. The officer ordered both inmates to stop fighting and get down, with no effect. The officer then deployed one 40mm exact impact round. The inmates stopped fighting but remained in an aggressive posture and refused to get down. The officer then deployed a second EIR; both inmates then complied with orders to get down and assume a prone position. Responding staff placed both inmates in handcuffs and escorted them to be medically evaluated. A lieutenant received an injury to his leg as a result of the incident.
As on June 17 – A suicidal inmate was kneeling down for the application of leg irons. When the cell door opened, the inmate suddenly began kicking in a backward motion, striking the officer in both forearms as well as the leg. The officer activated the unit alarm. Responding staff arrived and restrained the inmate on the floor and applied a spit hood. Staff then secured the inmate into a Stokes Litter, placed him on a gurney and escorted him from the section. The officer sustained abrasions and scratches to both forearms and the leg, and was taken to a local hospital for treatment.
June 13 – Two officers were escorting an inmate from the rotunda back to his cell when the inmate kicked one of the officers, striking him in the ankle. The inmate then got into a prone position. The officers maintained control of the inmate until responding staff arrived. No use of force was used and the inmate was escorted back to his cell without further incident.
June 11 – A CTC staff psychiatrist considered an inmate a danger to himself due to his continual banging of his head on the cell walls causing injuries, so he ordered placement into four point restraints and then forced medication. After numerous failed attempts by CTC staff to gain compliance of the inmate, an extraction team was assembled. Upon arrival, the CTC doctor declared a medical emergency and ordered the immediate removal of the inmate from his cell. A sergeant opened the door and the team entered the cell. The inmate immediately stood up, turned and faced the officers and lunged toward an officer who was holding an extraction shield. The officer used the shield and forced the inmate against the cell wall. The inmate reached around the shield and attempted to strike the officers with his fists. One of the officers was struck on the lip. The officer then forced the inmate to the ground. While on the floor, the inmate began kicking and twisting his body in an attempt to break away from the officers. Another officer used his expandable baton, with no effect on the inmate’s actions. Additional use of the baton had the desired effect and the inmate stopped struggling and assumed a prone position. Officers placed the inmate in restraints and a spit mask was placed over the inmate’s head.
Also on June 11 – An inmate refused to take his court-ordered medication, so an extraction team was formed, thus allowing an approximate one-hour cool down period. The inmate continued to refuse to comply with orders and remained at the back of the cell. A sergeant used pepper spray to gain compliance and the inmate was removed from the cell. The inmate was taken to the CTC shower area for decontamination but the inmate refused to decontaminate.
Also on June 11 – An inmate refused his medication and began to put on additional clothing, and covered his face. An officer ordered the inmate to stop preparing for an extraction, to which the inmate refused. The officer opened the cuff port and another officer dispensed pepper spray to gain compliance. The inmate was handcuffed, removed from his cell, escorted to the yard shower and allowed to decontaminate before being rehoused.
June 7 – Two Correctional Treatment Center officers were escorting an inmate in waist and ankle restraints back to his assigned cell. As they approached the cell, the inmate turned and spit at one of the escorting officers. The officer turned his head but was struck on the right side and back of the head. Both officers used physical force to bring the inmate to the floor and into a prone position. The other officer placed a spit hold over the inmate to prevent him from spitting again. Both officers assisted the inmate to a standing position and escorted him back to his assigned cell without further incident. The assaulted officer was medically evaluated, noted a swollen knee, and was transported to an outside hospital for further treatment.
June 6 – An inmate was received from Eel River Camp for battery on staff and possession of a cell phone. The inmate was housed in Ad Seg pending a transfer to another institution.
June 5 – An officer was escorting an inmate from the shower back to his assigned cell when the inmate attempted to kick the officer. No use of force was necessary because the inmate on his own went to the ground. Leg restraints were placed on the inmate, and he was escorted to be medically evaluated and back to his assigned cell.

March 17 - As inmates were exiting the law library, an inmate attempted to murder a correctional officer. The inmate stabbed the officer once in the neck. The weapon failed to penetrate the officer's neck and the inmate made a second attempt to stab the officer. However, the officer backed away and was able to spray the inmate with pepper spray; which stopped the attack. The weapon was manufactured from melted plastic and was sharpened to a point at one end.

February 25 – An inmate was being escorted when he became resistive, requiring staff to use physical force to place the inmate on the ground. The inmate received minor abrasions to the face, shoulder, hip and knee. Both officer were injured, one with pain to the shoulder and neck. The other officer sustained a deep abrasion to his hand with active bleeding.
February 25 – On the Unit B-6 yard three inmates were involved in a fight. When the inmates refused orders to stop fighting, staff fired one exact impact round, which did not strike any of the inmates. Following the discharge of the 40 mm, the inmates complied and laid down on the ground. Two of the inmates had minor abrasions and the third had no injuries. There were no staff injuries.
February 16 – An inmate refused orders to submit to handcuffs and exit his cell for a cell search. The inmate began ingesting contraband in an effort to keep staff from gaining access to the items. Upon ingesting the items, the inmate complied with staff orders and was placed into handcuffs and removed from his cell. While under escort, the inmate became physically resistive and the unit sergeant placed the inmate on the wall and ordered him to stop resisting. As the escort continued, the inmate forcefully pushed his body in a backward motion and began turning toward the escorting staff. The inmate was physically forced to the floor to stop his actions. During the incident, the inmate assaulted the sergeant by crushing his hand between the floor and the inmate's chin. The sergeant was medically examined and was found to have sustained a swollen hand. The sergeant was transported to an outside hospital for further evaluation. The inmate was medically examined and found to have sustained abrasions to his knee, elbow, torso, back of the head and eye.
February 5 – An inmate attempted to bite an escorting officer while under escort. Staff used physical force and placed the inmate on the ground to maintain control of him. During this incident there were no injuries to the escorting staff or the inmate. Two hours later, the same inmate was being escorted to the CTC, when he became physically resistive to the second team of escorting officers. Physical force was again utilized and staff placed the inmate on the ground to maintain control of him. One officer sustained pain to the back of the neck and swelling to a right knee, the other officer had pain to the upper back. The inmate had an injury to his hip and a cut in the mouth.
February 4 – An inmate kicked an escorting officer in the leg while being escorted to a group room. Staff used physical force to place the inmate on the ground. While on the ground, the inmate then attempted to spit on another officer who was responding to assist in controlling the inmate. The spit did not make contact with the officers. There were no injuries to staff as a result of this incident.
February 4 – An inmate attempted to barricade his cell door while being ordered to submit to handcuffs by the unit sergeant. When the sergeant attempted to pepper spray the inmate to keep him from barricading the door, the inmate grabbed the sergeant's arm. Both the sergeant and the inmate were exposed to pepper spray. Following the use of pepper spray, the inmate complied with orders and was handcuffed and removed from the cell. There were no injuries to staff or the inmate as a result of this incident.
February 2 – An inmate attempted to head butt an officer during escort to the shower. Staff used physical force and placed the inmate on the ground to stop his actions. The inmate was medically examined and re-housed without further incident. There were no injuries to staff or the inmate.

January 24 - A PSU inmate attempted to assault an officer by throwing an unknown liquid substance at her. The liquid did not strike the officer. There were no injuries.
January 22 - An ASU inmate spit on an officer when the officer and his partner were attempting to place handcuffs on the inmate. The spit struck the officer's face. The inmate then attempted to grab the officer through the cuff port, but was not successful. The officer was examined and refused further treatment.
January 19 - A PSU inmate head-butted an escorting officer in the cheek, after being removed from the exercise yard. Staff used physical force to place the inmate on the ground. With the assistance from responding staff, the inmate was medically examined and found to have no injuries. The officer was taken to an outside hospital, examined and released.
January 17 - A SHU inmate housed in Unit C-5 gassed an officer with an unknown liquid. The officer was struck in the face, jumpsuit and back of the head. Following the gassing, the inmate refused orders to submit to handcuffs and come out of the cell. A controlled use of force was initiated and followed by pepper spray. The inmate complied with orders and submitted to handcuffs. The officer was medically examined and it was noted that the unknown liquid did splash into the eyes of the officer.
January 14 - An inmate who was being escorted back to his assigned cell attempted to head butt an officer and staff used force to place the inmate on the ground. The inmate had an abrasion on his knee. One of the officers received a cut on his hand but remained at work.
January 8 - An inmate housed in Unit B-8 lit some clothing on fire in his cell. A staff member who was on the tier conducting count saw the fire, immediately removed the inmate from the cell and extinguished the fire. Due to the smoke in the section, all inmates were removed from the section and were seen by medical staff to ensure they had no medical issues. The smoke was cleared and all inmates were re-housed in the unit, with the exception of the inmate who started the fire. The inmate was placed in ASU. There were no injuries as a result of the incident.
January 6 - During escort to the shower, an inmate kicked an escorting officer, striking his right knee. Staff used physical force and placed the inmate on the ground to re-gain control of him. The inmate was not injured, was medically evaluated and re-housed in PSU. The officer had a reddened area on his knee but remained at work.
December 2008
December 11 – A riot involving 14 black and nine white inmates occurred on the main exercise yard. The riot started when a white inmate attacked another white inmate who affiliates himself with the Kumi Disruptive Group. Staff fired three Mini 14 rounds as warning shots, two Exact Impact rounds, expandable batons and various chemical agents to gain compliance with orders to stop fighting and get down on the ground. There were no major injuries to the inmates. There were no staff injuries.

June 7 - A riot broke out on an exercise yard. Verbal orders to stop fighting were ignored, so staff used pepper spray, rubber baton rounds and fired two rounds from a mini 14 rifle to stop the riot. Officers found four inmate-made weapons at the scene including one made of metal. No staff were injured.
June 3 - A PSU inmate who refused to take his court ordered mediation was extracted from his cell during a controlled use of force. Staff used one burst of pepper spray and the inmate complied with orders. The inmate was decontaminated, given his medication and re-housed.
May 2008
May 10 - Staff found an inmate intoxicated and being destructive to state property within his assigned cell. To secure the safety of his cell mate, staff used pepper spray to stop his actions. When staff proceeded to remove the inmate from the cell, he kicked the sergeant in the knee and an officer in the leg. During the escort to medical, the inmate spit on another officer.
May 6 - A SHU inmate being escorted to the SHU, kicked another inmate also being escorted. Staff used physical force to regain control of the assaultive inmate. Both inmates were medically evaluated and rehoused without further incident. One officer and one inmate sustained minor injuries as a result of the incident.

March 22 - An inmate attempted to barricade his cell. Staff used OC pepper spray on the inmate. The inmate then dipped his laundry bag in his toilet and began to swing it towards the Officers on the tier. Liquid from the bag struck one staff member on the arm. The inmate then used a cup containing unknown liquid throwing it from the cell, which struck the Officer in the arm. While the inmate was being given in-cell decontamination instructions, he was handcuffed and escorted to the CTC.
March 21 - Two inmates attacked another inmate on the Facility A Main Exercise Yard. Staff fired Exact Impact rounds in an effort to gain compliance to stop the attack. As staff were responding to the incident, the inmates stopped the attack and got down on the ground. The victim inmate received nine puncture wounds and eight lacerations. There were no staff injuries as a result of the incident.
March 6 - On the exercise yard, two inmates were involved in mutual combat. Three Exact Impact rounds were fired in an attempt to gain compliance with orders to stop fighting and get down. The inmates stopped fighting and were placed in handcuffs and re-housed.
March 5 - Two cellmates were involved in mutual combat on the tier. The inmates ignored orders to stop fighting and get down on the floor. Staff fired one 40mm Exact round to gain compliance from the inmates. The round missed the inmates; however it had the desired effect and the inmates stopped fighting, got down on the floor and were handcuffed by responding staff. The inmates were medically examined and re-housed.
March 24 - An inmate who was being escorted back to his cell from the shower kicked one of the escorting Officers. Staff used physical force to place the inmate on the floor and maintain control of him while additional staff responded to the incident. The inmate was medically evaluated and found to have no injuries. The Officer who was kicked was not injured.
March 19 - Two inmates attacked another inmate on the Main Exercise Yard of Facility B during yard recall. To gain compliance, staff used three Exact Impact rounds, OC pepper spray, physical force, and Expandable Batons. Two of the Exact Impact rounds hit their intended targets; the third round missed the intended target, but did not strike any inmates. All inmates involved had minor injuries. No staff injuries occurred as a result of the incident.
March 14 - In Unit 2A, two inmates were being escorted back to their cell following showers. Both inmates were placed inside their cell and when the handcuffs were removed from the first inmate, he attacked the second inmate with a razor blade, who was still in handcuffs. Staff immediately gave orders to stop the attack; to which the inmate did not comply. OC pepper spray was used and the inmate stopped the attack. The victim inmate was transported to the CTC, then to a hospital for additional treatment. The assailant was de-contaminated and re-housed. No staff were injured as a result of the incident.
March 13 - The Facility B8 Floor Officer was inside the Housing Unit office when a BMU inmate entered the office and proceeded to physically assault the Officer. The inmate hit the officer twice in the face. Staff used physical force and OC pepper spray to gain control of the combative inmate. The inmate was escorted from the area, decontaminated and found to have no injuries and placed in Administrative Segregation. The Officer was transported to the hospital for medical evaluation. Upon completion of the evaluation, the Officer returned to PBSP.
March 12 - Five inmates attacked two inmates in the dayroom. Staff orders to stop were ignored. Three Exact Impact Rounds were used to gain compliance. All inmates sustained minor injuries; there were no staff injuries as a result of the incident.

February 20 - During a controlled released, three inmates were involved in a fight. Staff used OC pepper spray to quell the incident. One inmate manufactured weapon was discovered. All inmates were exposed to OC and one inmate received minor injuries to his head. The other inmates were not injured. All involved inmates were re-housed in ASU. There were no staff injuries.
February 1 - An inmate leaving the showers turned and kicked one of the escorting officers in the leg. Staff immediately gained control of the inmate by placing him on the floor. The inmate was medically evaluated and returned to his cell without further incident. No officers were injured.

January 18 - Over 60 northern and southern Hispanic inmates were involved in a riot on Facility B. The riot occurred simultaneously in Education, the B-2 Dining Room, and the Main Yard. One Mini-14 round was fired as a warning shot. Exact impact rounds and OC were also used to quell the riot. Two inmates were transported by ambulance to outside hospitals although their injuries were not considered life threatening. Two staff members also suffered minor injuries.
January 15 - An inmate struck an officer in the face while being prepared for transport to the Department of Mental Health. Additional staff used an expandable baton and pepper spray to gain control of the inmate. The assaulted officer suffered some swelling and a cut on the nose.
