December 2011
December 29 – During a group disturbance on another hall, all wards were instructed to return to their rooms. All wards complied with the exception of one ward who refused and uttered unintelligible threats. As a PA1 stood blocking the stairway to the second tier, the ward forcefully pushed through the PA1’s left side causing her arm to get caught on the stair rail. This caused the PA1 to fall to a sitting position on the stairway. The PA1 suffered aching and redness to her left arm.

April 2011
April 7 – A YCC and Supervising Treatment Team Supervisor gave orders for a ward to stop throwing items and a warning that he could face a staff assault charge if he did not stop. The ward responded that he did not ‘give an expletive about a staff assault charge’. The ward appeared to become more upset and then reached into his cell and threw a Styrofoam cup filled with an unknown liquid substance at the face of the YCC. The substance covered the entire front and side of the YCC. The YCC was transported to medical and subsequently referred to an outside medical facility for treatment of exposure to an unknown substance.
February 2011
February 1 – Upon being denied a straight-through state paid phone call, a ward threw a 388-page book towards the YCC station, striking a YCC in the head. An alarm was initiated and the ward was secured in his room without further incident. The YCC was medically evaluated and remained on duty.
September 2010
September 30 – A ward wanted to talk to a YCC about an item that was removed from his room earlier in the day during a room search. The YCC was seated at a computer inside an office. As the ward entered the room, he began to strike him with closed fists in the face as the YCC was still seated. As he began to strike the YCC, another YCC activated his personal alarm and started toward the office along with another YCC. As they entered, the attacked YCC was able to get to his feet and grab the ward around his waist. The responding YCCs gave verbal orders to stop with negative results. The ward continued to strike the YCC on the back and sides. Together the three YCC were able to use physical force to stop the ward’s assault. The ward continued to struggle as they repeatedly gave orders to stop resisting. Two YCO arrived on the unit and proceeded to put the ward in mechanical restraints. The attacked YCC sustained a bloody nose, and bruising/swelling to the face.

August 27 – A ward gassed a staff member.
August 26 – While a ward was being questioned about an earlier assault, the ward stood up and spit into the face of a Lieutenant; the spittle also struck the face of another Lieutenant. The ward was placed in full restraints as the other Lt. activated his personal alarm and summoned for staff assistance. NAC security arrived and escorted the ward to secure quarters.
August 25 – A ward put soap in a sock and attacked an officer.
July 2010
July 31 – A youth correctional counselor was giving a ward instructions to uncover his door window when the ward yelled obscenities at the counselor. This confrontation continued for several seconds. The YCC requested a security chain. As the counselor applied the chain to the door handle, another counselor arrived and instructed the ward to remove the obstruction. The ward yelled more obscenities. The YCC instructed the ward to step away from the door while both YCCs attempted to open the door. When the door opened, the ward spit at one of the counselors, making direct contact with his chest.
July 8 – A youth correctional counselor instructed a ward to end his phone conversation to prepare for evening clean up. The ward ignored staff orders and continued his conversation. After being instructed several more times, the ward finally ended his phone call, but confronted the YCC about having to end the call. The counselor advised him that he would be provided another phone call later but the ward continued to express anger. The ward suddenly began striking the YCC in the face; the YCC used pepper spray in an attempt to stop the assault, but the ward continued his assault. The counselor then used physical force to stop the ward. Another counselor responded to the scene and placed the ward in restraints. The counselor was examined and sent to an outside medical facility for further treatment.
July 1 – Sacramento Hall was conducting program and several of the wards on program were becoming disrespectful and refusing to go their rooms. Staff responded to the scene and placed one ward in restraints. Though another ward became resistive while handcuffs were being placed on him, he was handcuffed and escorted to a secure room. Once in the room, the youth became verbally defiant as well as physically resistive to being placed in the room. The ward blocked the door with his body. Several of the officers used verbal commands to gain compliance, but the ward refused to comply. The ward was placed on his bunk in a prone position. Once compliance was gained, the restraints were removed and the officers began to exit the room. As the last officer released control of the ward, he jumped up and began assaulting the officer, striking him numerous times in the head, face and torso. The other officers reentered the room and used pepper spray to stop the assault.
June 2010
June 24 – As youth correctional counselors were responding to an attempted suicide, another ward near the incident began covering his window. The ward spit blood and saliva on a YCC’s boot and leg. The YCC attempted to gain control of the ward as he was becoming more resistive by thrashing his body and kicking his legs at staff. The ward kicked the counselor in the abdomen causing the counselor to fall backward, landing on the cement sink and bed. The counselor and a lieutenant used physical force to secure the ward in restraints and escort him from the building.
June 24 – A ward was covering his window with a mattress. When a counselor responded to the scene, the ward spit in the counselor’s face.
June 16 – Two wards approached each other in the middle of the dayroom. A counselor activated his personal alarm and ordered the wards to get on the ground. One ward threw a punch toward the other ward, but did not make contact. The non-aggressive ward complied with staff orders to get down but the other ward continued his assault and approached another ward. As the two wards began to fight, the attacking ward pulled a sock containing soap from his waist band and began swinging it at the ward. Pepper spray was used with positive results from one ward but the aggressive ward continued to swing his sock toward a counselor. More pepper spray was used with negative results. The ward then threw two trash cans at the counselor hitting him in the torso and midsection. Another ward pinned the aggressor against a wall and took him to the ground. The ward was placed in handcuffs and leg irons and escorted out of the building.
June 15 – A program was terminated because the wards were being defiant. As youth correctional counselors were handcuffing wards and escorting them out of the room, a ward grabbed at a counselor’s hand in an attempt to grab the restraints. The counselor was able to secure the restraints and pull back in an effort to get away from the hostile ward. As another counselor went toward another ward, the ward began cursing and yelling at the counselor. The ward then reached out of the tray slot and began slamming the slot down in an effort to damage state property. The counselor gave verbal orders to stop but the ward refused. The ward then spit out through the tray slot striking, the counselor. The ward then tried to spit at another counselor. The counselor used pepper spray to stop the ward’s actions. The counselor then tried to close the tray slot but it was damaged. The ward ran back to the open tray slot and pushed it down hitting the counselor’s finger, attempting to grab his hands. The ward finally complied with another verbal order to stop. The officer was able to secure the tray slot using physical force. The ward then covered his window with paper, and began throwing liquid substances out of an opening in the tray slot. A lieutenant began talking to the ward, as the ward squirted a liquid substance out of the opening of the tray slot, hitting the lieutenant. After about five minutes of talking with the ward, he complied with staff orders to uncover the window and was escorted from the room.
May 2010
May 18 – During dayroom program two wards began fighting. Verbal commands to stop were ignored, so staff used pepper spray to stop the combatants. Once the situation was contained, two youth correctional counselors were placing a ward in the shower for decontamination. At the same time the remaining wards were instructed to proceed to their rooms. One ward who was walking to his room, turned and ran to the shower, attacking a YCC by striking him in the head and face several times. Another YCC used physical force to stop the assault but not before being struck several times in the face. Two other YCCs who assisted in the restraint were also assaulted by the ward. The ward was placed in both handcuffs and leg restraints before being escorted to a security van where he continued to be resistive and out of control, hitting his head on the van door. He ignored orders to stop. The YCCs then opened the door and the ward kicked one in the chest. Another YCC then used pepper spray to stop the ward’s assault on the counselor. Once control of the ward was regained, he was transported to the infirmary. All five YCCs remained on duty.

MARCH 18 -- A ward had covered the window of his cell, prohibiting staff from accounting for his safety. As a counselor and another staff member opened the door to clear the obstruction, the ward grabbed the counselor’s arm, violently slamming her into the door and trying to pull her into the cell. The counselor was able to call for assistance via radio and used pepper spray, as another counselor pushed the ward back into the cell. After the incident, the ward continued to threaten the counselor by stating, “I’m going to get you!” The counselor received aid at an outside medical facility.

January 26 – A correctional counselor was assaulted by a ward. The officer was punched in the face and suffered a black eye.
January 12 – A ward, who was upset about his case conference, was playing dominoes in the dayroom. As a correctional counselor entered the dayroom, the ward threw the dominoes, ran across the room, and began swinging his arms and fists as he approached the officer. The officer managed to grab the ward around his torso but was struck twice in the face before other staff could respond and assist in gaining control of the ward. The correctional counselor was examined by medical and released.
January 2009
January 7 - While trying to get a ward to submit to being handcuffed on a living unit, the ward escaladed into a non-compliant state; challenging three officers and a Lieutenant. The ward advanced towards the Lieutenant with clinched fists and the ward was subsequently maced and physically subdued and restraints were applied. While transporting the ward to the lock up unit, he kicked out the side door window of the vehicle. While escorting the ward into the unit and to the showers for decontamination, the ward turned around and spat in the face of an officer. The ward then kicked the officer in the groin. The ward was then restrained to the floor to gain control of his actions. The officer sustained a sore groin and will go through a year of testing from being exposed to the ward's saliva.

March 5 - Two Officers were escorting a ward when the ward became resistant and kicked one of the officers. The ward and the Officer received medical attention.
