April 2012

April 27 – An officer was escorting a teacher to an inmate’s cell when he noticed an inmate on the top tier move to the dayroom table next to an officer. The inmate then stood up and aggressed toward the officer, striking the officer in the face. The officer brought the inmate to the ground and tried to apply restraints. As a Yard Officer arrived and assisted the attacked officer in restraining the inmate, the inmate grabbed hold of the officer’s groin. The two officers were able to restrain the inmate as other staff arrived. The officers were sent to TTA and then to an outside medical facility for treatment.

March 2012

 

March 21 – While an officer was cuffing up an inmate to be moved from a holding cell, the inmate turned a spit in the officer’s face.
 
March 10 – While conducting the lock down shower program in Facility C Building 3, an officer was escorting an inmate to the shower in restraints. During the escort the inmate spit directly into the officer’s face. The officer was taken to TTA for medical evaluation including a risk assessment for exposure. The officer was then taken to an outside medical facility for treatment of exposure.
 
March 7 – An inmate was being interviewed by a sergeant when the inmate spun around and kicked the sergeant three times in the leg. The sergeant was taken to TTA for medical treatment and returned to his post.
February 2012

 

February 7 – As staff were processing inmates through Bravo work change, an officer was attempting to place one of the inmates in handcuffs. The inmate, who is a Blood, turned and punched the officer in the face. Three other inmates, who were Crips, shoved an officer as she was trying to secure the door. The inmates were able to assault two officers and a sergeant. Responding staff arrived and gained control of the inmates. Staff was escorted to CTC and given medical treatment and then taken to outside medical for further treatment. An officer sustained two black eyes and a laceration below the ear. Two officers and the sergeant sustained bruises and exposure to pepper spray.
January 2012

 

January 15 – An officer was removing an inmate from his cell. As the cell door opened the inmate turned around and kicked the officer in the lower leg and attempted to grab the officer by the jumpsuit. The officer gained control of the inmate with assistance from responding staff. The officer sustained bruising and soreness to the legs. The officer was treated at the CTC.
  
January 11 – 300 inmates began rioting on the Facility A yard. Officers responded and used less lethal rounds, pepper spray and two warning shots to stop the riot. Several inmates received stab, puncture and slashing type wounds. No staff were injured.
December 2011

 December 9 – An officer was working the Bravo Yard Ad Seg when he was gassed. Specifically while the officer was preparing to cuff an inmate through the cuffing port in the shower, the inmate turned and threw feces in the officer’s face. The feces made contact with the officer’s eyes, ear and mouth. The officer was taken to outside medical for treatment.

September 2011

 

September 30- An officer was performing his duties as Ad/Seg search and escort officer, when he was assaulted by an inmate. The officer sustained an injury to his left elbow and was taken to outside medical for evaluation.
July 2011

 

July 30- An officer was escorting an inmate from Bravo 1 ASU to TTA, when the inmate turned towards the officer aggressively to batter him. The inmate’s right shoulder hit the officer in the chest. At the same time the inmate forcefully threw his head back in an attempt to head butt the officer. The officer then took the inmate to the ground and injured his left elbow as a result of the assault. He was sent to TTA for medical evaluation. The officer was then sent to DRMC and given further medical treatment.
April 2011

April 28 – An officer was picking up trays when an inmate threw urine at the officer, striking him on the skin and jumpsuit. The officer refused medical treatment and was relieved of his shift.


April 27 – An officer was conducting random searches of inmates leaving the dining hall when an inmate threw his state-issued clothing at him, striking the officer in the chest.


April 20 – An inmate assaulted two officers. Pepper spray was used to stop the inmate’s actions. One officer sustained scratches and bruising to his forearms, injury to his eye and pepper spray exposure. The other officer sustained scratches and bruising to her forearms and exposure of pepper spray.


April 19 – An officer was un-cuffing an inmate on Facility D Building 3 when the inmate turned and struck the officer in the forearm. The officer regained control of the inmate with the help of another officer, placing the inmate back into restraints. The officer sustained redness to the forearm.

January 2011

January 20 – An officer was feeding morning chow at CTC when he went to collect a tray from an inmate. The inmate had a cup of liquid hidden under the tray and threw the liquid at the officer. The liquid did not make contact with the officer. It was later determined the substance was urine.

January 6 – An officer was instructing an inmate to cuff up, the inmate began yelling obscenities and told the officer to pepper spray him. The inmate then started to bang his head on the side of the holding cell. When the officer moved toward the inmate he lunged toward the officer, striking the officer’s shin with his leg.
 

December 2010

December 16 – While placing a restrained inmate into his cell, the inmate used his arm to clamp down on the officers right hand. The inmate then spun and deliberately pulled the officer to the ground. The officer sustained injury to his hand and back spasms.

September 2010

September 29 – A Yard Clinic officer observed two Southern Hispanic inmates attack a black inmate. One of the assailants grabbed the attacked inmate’s waist restraints while the other inmate began stabbing the inmate numerous times in the neck and back. A personal alarm was activated and the inmates were ordered to stop and get down with negative results. One officer used a baton striking one of the assailant inmates with negative results. The inmate continued his attack which necessitated the officer to strike the inmate a second time. All inmates complied with orders to get down. An inmate-manufactured weapon was discovered and placed into evidence. 

August 2010

August 27 – Three Southern Hispanic inmates attacked another Southern Hispanic inmate in the torso and face. The attacking inmates were given numerous orders to stop with negative results. Officers used three 40 mm wood baton rounds and pepper spray to stop the attack. The attacked inmate sustained multiple injuries consistent with a weapon. One weapon was discovered near the suspects. The inmate was taken to an outside hospital for treatment of his injuries. All involved inmates were rehoused in Ad Seg.

June 2010

June 28 – As an officer was applying handcuffs to a disruptive inmate, the inmate spun around and hit the officer in the eye. The inmate was subdued by responding staff.

June 23 - During an escort, an inmate jumped out of his wheelchair and attempted to assault the escorting officer. Physical force was used to restrain the inmate.

Also on June 23 - An attempted homicide with a weapon occurred on Facility B upper yard.  A Mexican inmate repeatedly stabbed another inmate.  Staff used two 40mm direct impact rounds to stop the attack. The injured inmate was transported to an outside hospital.

Also on June 23 - A female officer was conducting count in Facility C, Building 5. During the count the officer looked into a cell and saw the inmate lying on top of his blankets, unclothed and masturbating. The officer flashed her light but the inmate continued to masturbate. The inmate was escorted from the building.

June 22 - Staff discovered an inmate in Ad Seg had covered all windows of the cell, preventing staff the ability to ascertain the welfare of the inmate. The sergeant used a shield for safety and opened the cell port. The inmate was unresponsive and the unit alarm was sounded. Staff conducted an Emergency Medical cell entry. Once the inmate was restrained, he became responsive. The inmate had abrasions, scratches and dried blood on his forearm, indicating self-mutilation.

Also on June 22 - As inmate was being escorted, he turned and spit on the escorting officer.  Staff used physical force to subdue the inmate.  

June 21 - An inmate was the victim of a battery by three other inmates.  The Yard Gun officer used his 40mm and yard staff used their batons and pepper spray to stop the attack.  The victim was transported to an outside hospital.

Also on June 21 - An inmate was attacked by two other inmates using weapons.  The attacking inmates complied with staff orders to get down.  The injured inmate was taken to an outside hospital; the assailants were charged with attempted murder.

June 17 - While an officer was escorting a nurse on the tier for “pill pass” an inmate informed staff that his cellmate was having problems breathing.  The cellmate admitted to having chest pains.  Upon pulling the inmate out of the cell it was discovered the inmate had slash and puncture wounds on his back, chest, and abdomen, indicating he had been assaulted with a weapon.

Also on June 17 - An inmate was refusing to return to his assigned cell. An officer approached him and ordered him to cuff up.   The inmate complied and the officer took the inmate back to his cell. Upon removing the restraints, the inmate spun around and struck the officer in the mouth with his elbow.  The officer used physical force to restrain the inmate until responding staff arrived.  

Also on June 17 - An inmate was throwing a football at the yard gun officer, making verbal threats.  Staff approached the inmate and ordered him to stop and submit to handcuffs.  The inmate clenched his fists and began to advance towards the officers.  The yard gun officer used the 40mm to stop the threat toward the officers.

June 15 – When an inmate was seen lying down on the yard, the yard was put down. Upon investigation, the inmate was discovered to be unconscious, and had been battered by unknown assailants.

June 14 - An inmate committed the act of indecent exposure when he stood in front of his cell door window, looking toward the control booth officer, and began to masturbate.

June 13 - An officer noticed an inmate-manufactured weapon on the floor in front of a cell. Approaching the cell to retrieve the weapon, he noticed the inmates inside the cell fighting.  The officer used pepper spray in order to stop the fight; one of the inmates had injuries indicating a weapon had been used.

June 11 - In housing unit B2’s concrete yard, three inmates attacked another inmate. One 40mm direct impact round was used to stop the attack.

Also on June 11 - Two inmates began to fight inside a CTC holding cell, requiring officers to use pepper spray to stop the violence.

Also on June 11 - Ten inmates affiliated with the BayAarea 415 on Facility A lower yard were involved in a riot that resulted in a Code 2 response.  A 40 millimeter multi-launcher and pepper spray grenades were used to stop the riot.

Also on June 11 - Two inmates began to fight on C Facility, requiring staff to use the 40mm direct impact weapon to stop the violence.

June 10 - An inmate on B facility began cutting himself on the arm; officers responded and took custody of the inmate.

Also on June 10 - On B Facility, an inmate refused to return his food tray from the morning meal.  Officers were given clearance and performed a tactical cell extraction to gain compliance.     

June 9 –An officer was attempting to place handcuffs on an inmate when the inmate grabbed the officer’s hand and pulled it through the food port.  The officer used pepper spray to stop the inmate’s actions. The inmate released the officer’s hand and threw a bar of soap out of his cell, striking another officer in the stomach.

Also on June 9 – During the afternoon medication pass an officer saw an inmate sitting on his cell floor. His face was swollen, he was bloodied, and the cellmate was standing in the back of the cell. The officer ordered the cellmate to the food port and placed him in restraints. The inmate on the floor was in a semi-conscientious state, his head severely swollen from an apparent battery by his cellmate. The inmate later died from his injuries at an outside hospital.

Also on June 9 - On Facility B, three inmates began attacking another inmate. The inmates refused to get down, so two 40 mm rounds were discharged and all inmates assumed a prone position.

June 8 –As a counselor was exiting the lower yard on B Facility, two Northern Hispanic inmates rushed the pedestrian gate in an attempt to breach the patio when they made eye contact with two Southern Hispanic inmates. A sergeant was onsite and observed the Northerners make direct eye contact with two Southerners on the patio and quickly ordered the inmates to get down.  The inmates complied and the pedestrian gate was quickly closed behind the counselor.   The sergeant ordered the yard and inmates to be searched. Two Northern Hispanic inmates did not clear the metal detector, therefore they were placed on contraband watch.

Also on June 8 – Facility A staff responded to a Code 1 fight between two inmates on the upper yard. Both inmates did not comply with orders to get down requiring the use of pepper spray and a 40mm to stop the incident.

June 7 – An inmate refused to go into his cell.  Responding staff arrived and ordered the inmate into the cell; still, he refused.  A sergeant handcuffed the inmate and escorted him to his cell. The inmate refused to go into the cell and turned and kicked the sergeant in the shin. The sergeant forced the inmate to the ground while another sergeant used pepper spray, striking the inmate in the face.  The inmate was escorted to the program office without further incident.  

June 3 – A riot broke out on Facility A upper yard involving white inmates and other inmates. Officers immediately called a Code 2 via institutional radio and ordered the yard to get down. Responding staff used pepper spray and deployed two CN tactical grenades to stop the disturbance on the yard.

June 2 – A riot involving the North and South disruptive group broke out on a yard. Inmates were seen striking each other over the head with state-issued canes.  One Southern Hispanic, who was stabbed in the neck, was taken to outside medical facility for further treatment via ambulance, Code 3.  Staff used chemical agents to stop the riot.

Also on June 2 – Facility D staff saw four Southern Mexican inmates attacking another on the Facility D middle exercise yard. At the same time, Facility D staff saw two other Southern Mexican inmates attacking another on the Facility D middle exercise yard. Staff used CN Grenades and 40mm rounds to stop the fighting. An inmate-manufactured weapon was found at the scene.

Also on June 2 – An inmate cut his arm between his elbow and wrist while in the shower. Staff recovered the state-issued razor blade the inmate used to cut himself, and he was transported to an outside hospital for treatment.

Also on June 2 – On Facility B two inmates were observed striking each other in the face and upper torso on the exercise yard, during yard recall.  Officers used chemical agents to stop the violence.

June 1 – Facility C staff was returning an inmate to his cell when his cellmate rushed out and attacked him. Staff used pepper spray and 40mm to stop the violence.
 

May 2010


MAY 27 --  A riot broke out on Facility A lower yard involving 13 East Coast Crips. Both observation officers used their 40 mm launchers to stop the disturbance.

Also on MAY 27 --  Facility D inmates attempted to batter an officer as he approached their cell to perform a search.  Officers used pepper spray to stop the attack.

And again on MAY 27 --  While D gym inmates were walking to D1 dining, three inmates battered another inmate. A 40mm direct impact round and pepper spray was used to stop the attack.

Yet another MAY 27 incident--  An inmate in the Facility D library began stabbing another inmate on the head and neck with an inmate-manufactured weapon. The weapon was made from flat metal, about five and a half inches long and more than an inch wide; one end was sharpened to a point and the other end had a white handle (possibly made of cloth). The injured inmate was taken out Code 3 to the hospital; the assailant is expected to be charged with attempted murder.  

MAY 25 --  Facility B staff saw two inmates attacking another inmate. Officers used pepper spray and 40mm to stop the attack.  One of the inmates became resistive to staff, injuring an officer. 

MAY 24 --  Facility A staff responded to a fight on the upper yard between two inmates. Staff used pepper spray to stop the violence.

Also on MAY 24 --  Facility C staff saw two inmates involved in a fight in Building 8.  Both inmates refused to comply with officers' orders to get down so pepper spray was used to the fighting. 

MAY 23 --  Facility C staff responded to a call for "Man Down" and observed an inmate cutting his left wrist with a state-issued razor blade. Facility C staff announced a Code 1 via institution radio, sounded the housing unit alarm and subdued the inmate. He was later sent to an outside hospital. 

MAY 22 --  An inmate exiting his cell for medication pass became irate and was ordered back to his cell. He started to yell obscenities at the LVN. As an officer began to escort the inmate back to his cell, the inmate stepped in front of the officer, hitting the officer in the chest. Officers used pepper spray to stop the attack. While the inmate was being escorted to the facility patio, he attempted to spit on another officer. Subsequently, a protective mask was placed on the inmate.

MAY 18 --  Facility D staff saw an inmate lying down on the ground in the handball court. Staff noticed the inmate not breathing, so they instructed officers to perform CPR on the inmate via chest compression until medical staff arrived.  Once a pulse was felt and the inmate started breathing again he was immediately transported to CTC and then to an outside hospital.

Also on MAY 18 --   An inmate committed battery on a peace officer -- the same officer who had only minutes before saved the life of the inmate in the May 18 incident noted above. During a pat down, the inmate spun around and struck one of the officers.  The officer took the inmate to the ground and the inmate continued to fight.  Responding staff were able to help stop the attack. The assaulted officer was taken to an outside hospital for treatment of  a broken hand.

Also on MAY 18 --  Inmates on Facility C were observed fighting.  Responding staff used pepper spray to stop the violence.

Also on MAY 18 --  An inmate committed an act of Indecent Exposure/Masturbating toward an LVN during pill pass.

MAY 17 --  An officer found an inmate-manufactured weapon in an inmate’s shoe while conducting metal detector sweeps of inmates entering the yard. The weapon was made of ridged metal about one inch wide by six inches long, sharpened to a point on one end with a cloth handle tied to the other end. The weapon appeared to be aluminum and cut from a crutch.

Also on MAY 17 --  An officer was conducting a clothed-body search on an inmate before going to his medical appointment.  During this search the officer felt a hard object along the inmate’s waistline. The inmate took off running away from the officer and toward the urinals on the lower yard; once there the inmate threw the object into the urinal and flushed it. 

Again on MAY 17 --  Officers were attempting to remove an inmate from the holding cell in Building 8, Facility C after he refused to return to his assigned cell.  The  inmate was placed in handcuffs, and as officers attempted to close the holding cell cuff port door, the inmate put his face to the cuff port and spit at the officers, striking both officers in the chest, arms, and legs.  The sergeant ordered a spit mask to be placed on the inmate.  During the escort, the inmate became resistive, attempting to kick the escorting officers and break free of their hold. The officers used force to take the inmate to the ground in an attempt to stop his aggressive action and regain control.

MAY 14 --  During yard, two inmates began to attack another inmate by striking him the face with their fists. The yard gunner fired two 40 mm rounds to gain control of the situation.

Also on MAY 14 --  The Facility B clinic officer saw an inmate who was assigned as the clinic porter attempt to secrete unknown objects into his rectum. The officer called for assistance. A subsequent search revealed the inmate was in possession of 15 TB syringes and two inmate kites. During the unclothed-body search, staff discovered two more TB syringes lodged between his buttocks. The AOD was contacted by the facility lieutenant. While searching the medical clinic, staff discovered 48 more TB syringes hidden inside a tub of powdered bleach.


December 2009

December 14 – An officer was attempting to give an inmate his food tray, when without warning the inmate threw his tray and its contents at the officer striking him on the torso. Simultaneously, the inmate reached through the food port and grabbed the officer’s arm and pulled it through the food port injuring the officer in the process. The officer used pepper spray striking the inmate in the face. The pepper spray had the desired effect and the inmate released his grasp on the officer. The officer closed the food port and responding staff placed the inmate in restraints and removed him from his cell for decontamination. The officer sustained a cut on his forearm.

March 2009

March 18 - During yard recall, 10 Northern Hispanic inmates attacked 28 Southern Hispanic inmates. An officer shot one warning shot and four additional shots with a mini 14. Four Southern inmates were shot. One inmate died from stab wounds from an inmate weapon. Two officers fired a 40mm approximately 8 times. There were no staff injuries. 

February 2009

February 25 – In Facility C Building 3 a Code 1 alarm was activated due to a fight in a cell occupied by two inmates. One inmate sustained two puncture wounds to the neck and chest. The inmate was treated for his injuries. No staff were injured as a result of this incident.

February 20 – An inmate informed staff that he was assaulted with a weapon on third watch during dayroom recall. The inmate showed staff the four-inch inmate manufactured weapon that was lying under the second step of the stairwell. The inmate received medical treatment and was re-housed. The inmate received minor injuries consistent with being the victim of a battery.

February 9 – After attending a scheduled medical appointment, transportation staff were escorting an inmate to an awaiting van. As the escorting officer opened the van door to allow the inmate to enter the vehicle, the inmate removed his leg restraints and one handcuff and ran from the area in an attempt to escape. The escorting officer and his partner pursued the inmate and tackled him to the ground. Two additional unarmed officers arrived on the scene and assisted the pursuing officers in placing the inmate into handcuffs. One of the officers who tackled the inmate sustained an injury to his hand; there were no other staff injuries. There were no injuries to the inmate.

February 5 – A dining hall worker in Facility B Dining 3 battered an officer inside the dining hall. While attempting to defend himself, the officer managed to activate his personal alarm device and alerted responding staff to assist him. Responding staff arrived and placed the inmate in restraints. The inmate was escorted for a medical evaluation. The inmate received minor facial/body injuries. The officer was taken to TTA for immediate medical attention and was subsequently transported to an outside hospital for further medical treatment. 

January 2009

January 26 - Staff on the Facility A lower exercise yard saw two inmates batter another inmate with closed fists; striking the inmate in the face and upper torso. Responding staff gave several verbal orders for the inmates to stop the attack and assume a prone position on the ground; to which the assailants complied. As responding staff got closer to the crime scene, the combatants got up and began fighting again. Staff dispersed a grenade into the middle of the combatants and used pepper spray. Staff observed one of the combatants holding a weapon. Staff gave the inmate several orders to drop the weapon and move away from it. As staff were gaining control of this incident, another inmate came running across the yard towards the scene, staff ordered the inmate to get down. As the responding supervisor arrived to the scene he saw one of the inmate throw an object away from his body. The item was one inmate-manufactured weapon made of plastic measuring 6 inches in length, 1 inch wide with one end sharpened to a point. The three combatants and the victim inmate were placed into handcuffs and escorted to the medical clinic. All four inmates were medically cleared and re-housed in ASU. There were no injuries to staff.

January 25 - An inmate committed the act of battery on another inmate with a weapon. As a result of the attack the victim inmate sustained a one-inch long laceration to his forearm. The assailant inmate sustained a laceration to his index finger.

January 24 - While attempting to take an inmate's blood pressure, the inmate reached up and grabbed and fondled the breast of the CNA. She immediately pulled away from the inmate and ordered him to stop. She reported the incident to the two correctional officers who were working in the unit. There was no force used by staff in this incident.

January 22 - As inmates were going to be processed into a substance abuse program, staff saw two inmates fighting. The observation officer ordered all inmates to assume a prone position on the ground; all inmates complied except the combatants. Responding staff gave numerous orders to stop fighting and assume a prone position. The inmates continued to ignore these orders and continued fighting. Staff used pepper spray and the MEB to quell the incident. One of the inmates was transported to the treatment center and subsequently sent to an outside hospital for treatment. The inmate sustained a laceration to the face and a dislocated shoulder. The inmate was returned to the institution and re-housed in ASU. The other inmate was medically cleared and re-housed in ASU. There were no injuries to staff.

January 22 - The control booth officer asked the housing unit officer to escort an inmate who refused to get off of the inmate telephone to his assigned cell. As the two officers approached the inmate, the inmate became argumentative. The inmate started to walk towards his cell, but as the officers got closer to the open cell door the inmate turned towards one of the officer and grabbed the officer's institutional radio microphone and the upper part of his jumpsuit. The two officers used physical force to force the inmate to the ground. The inmate was medically evaluated and cleared. There were no injuries to staff.

January 17 - Staff saw two southern Hispanic inmates striking another southern Hispanic inmate in the head and upper body. Responding staff saw a long, pointed metal object in the hand of one of the inmates as he was making stabbing motions to the back of the victim inmate. As staff responded the victim inmate broke free from the grasp of his assailants and attempted to run away. Staff witnessed one of the assailant inmates throw the object over the fence into the Vocational Access Gate area. All three inmates assumed a prone position on the ground without staff having to use force. The involved inmates were placed into handcuffs and escorted to the facility medical clinic. During the subsequent search of the vocational area, staff discovered an inmate-manufactured weapon measuring 6 ½ inches in length, ¼ inch in diameter, sharpened to a point on one end with a cloth handle on the other end. The two assailants sustained no serious injuries and were medically cleared and re-housed in ASU. The victim inmate sustained several puncture wounds to his back, arm, shoulder and knees.

January 7 - Two correctional officers assigned to the Facility D Gym were battered by several Southern Hispanic inmates. One inmate attempted to exit the gym to the yard but was stopped by one of the officer, this encounter quickly escalated to the inmate battering the officer. The other floor officer ordered the gymnasium to get down and responded to the scene to assist his partner. While responding to the scene the officer initiated an alarm response by announcing the incident over his state issued radio. Upon arriving to assist his partner, several other Hispanic inmates attacked the responding officer. As the officers attempted to defend themselves the inmate struck them with fists and repeatedly kicked them. Responding facility staff entered the gym and used pepper spray to get the involved inmates to stop their attack and assume a prone position on the ground. A code II alarm response was initiated and subsequently elevated to a code III response. The two officers were taken to the Correctional Treatment Center and subsequently transported to Delano Regional Medical Center where they received further treatment. 11 inmates have been positively identified as being involved in this incident and have been re-housed in ASU.

December 2008

December 23 – A riot involving 13 inmates, 10 Hispanics, two whites and one black, occurred on the facility C lower yard. Staff saw the group of inmates fighting on the black top and the control booth officer discharged one direct impact round. Responding staff arrived and used pepper spray to quell the riot. All involved inmates were medical evaluated and re-housed in ASU. There were no staff or other inmate injuries as a result of this incident.

December 22 – During dayroom activities, the unit officer observed two inmates striking each other in the head and facial area; the officer activated their personal alarm. The control booth officer ordered all inmates to assume a prone position on the ground, all inmates complied expect one of the combatant inmates who walked over toward the lower tier shower and threw an object onto the top tier. The inmate then assumed a prone position on the ground. Responding staff entered the section and placed both inmates into handcuffs and escorted them from the building. One inmate-manufactured weapon was discovered. It was approximately seven inches in length, wrapped with cloth and a rubber glove on one end to form a handle; the other end was sharpened to point. One inmate sustained several stab wounds to his arms and head and the other inmate sustained scratches and cuts to his hands.

December 21 – Three inmates committed the act of "Assault on an Inmate Attempted Homicide" against an inmate on the Facility D middle yard. The facility yard officer saw three inmates chasing the victim inmate striking him in the head and upper body; the yard and observation officer gave verbal orders for the inmates on the yard to assume a prone position on the ground, all inmates complied except the combatants. The yard officer dispensed one pepper spray grenade near the combatants. This had the desired effect as all involved inmates assumed a prone position on the ground. Responding staff placed the inmates in handcuffs; the three assailants were placed into holding cells while the victim inmate was transported to the Correctional Treatment Center and subsequently to an outside hospital for further medical treatment. The victim inmate sustained puncture wounds to his chest, abdomen, back of head and neck and a fractured jaw. No staff were injured.

June 2008

June 3 - In a dining hall, three inmates committed battery on an inmate with a weapon resulting in serious bodily injury. The Supervising Cook activated her personal alarm. Responding staff gave repeated verbal orders for the inmates to stop fighting and assume a prone position. The inmates ignored the orders and continued their assault on the victim inmate. Staff utilized pepper spray striking them in the facial area. This had the desired effect as all the inmates stopped fighting and assumed a prone position. The involved inmates were handcuffed and escorted to be decontaminated, medically evaluated and re-housed within ASU. The victim sustained approximately seven puncture wounds to his back, side and leg. The inmate was subsequently taken to an outside hospital for treatment.

May 2008

May 18 - An inmate committed battery on an officer. When staff opened the food port of a cell, an inmate reached through the open food port and grabbed the officer's duty belt and pulled the officer in towards the cell door. Staff utilized pepper spray to gain compliance with orders to release the officer. The inmate complied with staff orders to submit to being placed in handcuffs. The inmate was re-housed in ASU. The officer sustained no injuries and remained on duty to complete his assigned shift.

May 9 - Yard staff observed three inmates starting to batter another inmate. The control booth officer ordered the yard down and all inmates complied except the combatants. Responding staff formed a skirmish line near the incident and gave verbal orders for the combatants to stop fighting and assume a prone position on the ground. The combatants then complied with staff orders. The three inmates were placed into handcuffs and medically evaluated, cleared and rehoused in Ad Seg. Staff found a screwdriver, more than five inches in length and sharpened to a point with a three-inch cloth handle at the other end. All four inmates were identified as Southern Hispanics.

May 2 - In a dining room, seven Hispanic inmates were involved in a riot. Staff utilized pepper spray and 40mm to stop the riot. The inmates were medically evaluated and had only minor injuries. There were no injuries to staff. The inmates were rehoused in Ad Seg. A preliminary investigation revealed that the disturbance was related to drug debts.

May 2 - While at the Kern County Courthouse, an inmate committed battery against a transportation officer. The inmate made derogatory statements and then spat on the officer.

March 30 - Two inmates attacked another inmate by repeatedly striking the inmate in the chest and face. The housing unit Control Booth Officer discharged one Direct Impact round, striking one of the inmates in the hand; this had the desired effect as all inmates stopped fighting and assumed a prone position on the ground. Responding staff placed the three inmates in handcuffs and escorted them to the program area where they were medically evaluated, cleared and re-housed in ASU. There were no injuries to staff as a result of this incident.

March 29 - During the morning meal, two inmates began to attack another inmate striking his head and torso. The Control Booth Officer discharged one Direct Impact round and did not strike his intended target, but had the desired effect as the inmates stopped fighting and assumed a prone position on the ground. There were no injuries to staff or other inmates.

March 26 - Southern Hispanic inmates from seven different cells refused to comply with bed moves and covered their cell windows and doors with sheets, blankets and paper, making it impossible for staff to see into the cell. The on-site Incident Commander received authorization to conduct tactical cell extractions. The Incident Commander prepared to utilize a calculated use of force and assembled four cell extraction teams. Staff systematically followed cell extraction procedures and approached one cell at a time and gave verbal orders for the inmates to submit to being placed into handcuffs and exit the cell; the inmates refused and OC pepper spray was administered into each cell. The inmates complied with staff orders and submitted to being placed into handcuffs and removed from their cells. A total of 15 cells, 28 inmates, were involved in the incident. There was no physical force used or injuries to staff. All inmates were decontaminated, medically cleared and re-housed in ASU without further incident.

March 26 - A Medical Clinic Officer opened a holding cell door to allow another inmate into the holding cell; an inmate rushed through the open door, hitting the officer, pushing her backwards out of the way. The inmate then attacked another inmate, who was sitting in a chair in handcuffs. The inmate stabbed the victim inmate in the upper lip with an ink pen. The Officer gave verbal orders to get down while securing the holding cell door. The inmate complied and assumed a prone position. Responding staff placed the inmate in handcuffs and escorted him to the program area. The victim inmate was treated in the medical clinic and subsequently escorted to the Triage and Treatment area where he received sutures to close the wound. The assailant inmate was re-housed in ASU. There was no use of force or injuries to staff.

March 20 - Confidential information was received that a Correctional Officer was targeted for assault with a weapon by various members of the Southern Hispanic disruptive group housed in Administrative Segregation. Acting on the information, cell searches were conducted and a total of 17 altered razors were discovered.

March 19 - Two inmates were involved in mutual combat resulting in the use of OC pepper spray and one Stinger round.

February 2008

February 20 - During evening meal, an Officer observed two inmates involved in mutual combat. The Officer ordered the inmates to stop fighting with negative results. The Officer used OC pepper spray striking both inmates. The inmates stopped fighting and assumed a prone position. Responding staff placed the inmates in handcuffs with no further incident. Both inmates were medically cleared and re-housed.

February 15 - Two Southern Hispanic inmates committed battery on another Southern Hispanic inmate on the lower exercise yard. The observation Officer fired two direct impact rounds from his 40mm launcher; the rounds did not hit any inmates but had the desired effect of stopping the attack. All inmates were medically cleared and re-housed in ASU.

February 15 - Two inmates were involved in mutual combat. The Control Booth Officer fired one direct impact round from his 40mm launcher; the round did not strike any inmates but stopped the fighting and the inmates assumed a prone position. The inmates were medically cleared and re-housed.

February 13 - Two inmates in a Housing Unit engaged in mutual combat. Staff discharged one Direct Impact round from the 40mm launcher to quell the incident. No injuries were reported.

February 13 - An inmate who was disruptive in a vocational area was ordered back to his housing unit where he then swung at a Sergeant. The Sergeant put his hands up to defend himself from the attack and sustained two lacerations to his hand. Staff used OC pepper spray to subdue the inmate. The inmate was placed into restraints; while conducting an unclothed body search, staff discovered a 6 to 6-and-a-half inch inmate made weapon secured to the inside of the inmate's thigh.

January 2008

January 20 - An unidentified white female, approximately 30-35 years old, threw liquid from a "Big Gulp" cup on a correctional officer stationed at the gate house. The officer had approached a white Nissan Altima to determine how many visitor passes the passengers needed. The driver responded "Passes? I got your passes bitch," then threw the contents of her cup into the face of the officer. The driver sped away, executed a U-turn and left the institution. The vehicle was a newer car and had no plates. After an evaluation, the officer chose to remain on duty.

January 19 - A southern Hispanic inmate suffered a punctured lung after being stabbed 17 times by two other southern Hispanic inmates. The stabbing occurred as the inmates returned to their housing unit after the evening meal. On direct impact round was fired to stop the attack. The victim inmate was transported to an outside hospital.

January 17 - an inmate being escorted to Administrative Segregation spit in the face of an escorting officer. The inmate was taken to the ground to gain control. The officer remained on duty and completed his shift.

January 16 - An inmate was found to be bleeding from an injury to his forehead in Administrative Segregation. The inmate claimed that his cellmate had hit him in the head with a cup. The inmates were re-housed in separate cells.

January 14 - In the Administrative Segregation Unit two inmates were discovered to have injuries from being in a fight. After the discovery, the inmates again began fighting. Responding staff utilized pepper spray to stop the fight.

January 11 - An inmate was placed in a crisis bed after kicking a nurse and attempting to bite the finger of an officer while resisting medication mandated under a Kayhea order.
