May 2008


May 10 - Staff found an inmate intoxicated and being destructive to state property within his assigned cell. To secure the safety of his cell mate, staff used pepper spray to stop his actions. When staff proceeded to remove the inmate from the cell, he kicked the sergeant in the knee and an officer in the leg. During the escort to medical, the inmate spit on another officer.

May 6 - A SHU inmate being escorted to the SHU, kicked another inmate also being escorted. Staff used physical force to regain control of the assaultive inmate. Both inmates were medically evaluated and rehoused without further incident. One officer and one inmate sustained minor injuries as a result of the incident.

April 2008


April 7 - A PSU inmate assaulted an Officer while he was under escort. The inmate kicked the escorting Officer and the Officer placed the inmate on the ground. The inmate was wearing leg irons and has a lengthy history of staff assaults.

April 6 - In A Section dayroom of Unit B-6, two inmates attacked another inmate. While in the process of conducting the unit barber program, the Control Booth Officer released the victim for a haircut. A fight broke out between the inmates. The inmates refused verbal orders to stop and the control booth Officer fired one exact impact round to stop the attack. The round struck the suspect in the back. All of the inmates complied with orders to get down on the floor. The victim received a cut to the head. There were no staff injuries. All inmates were re-housed in Ad-Seg.

March 2008


March 24 - An inmate who was being escorted back to his cell from the shower kicked one of the escorting Officers. Staff used physical force to place the inmate on the floor and maintain control of him while additional staff responded to the incident. The inmate was medically evaluated and found to have no injuries. The Officer who was kicked was not injured.

March 22 - An inmate attempted to barricade his cell. Staff used OC pepper spray on the inmate. The inmate then dipped his laundry bag in his toilet and began to swing it towards the Officers on the tier. Liquid from the bag struck one staff member on the arm. The inmate then used a cup containing unknown liquid throwing it from the cell, which struck the Officer in the arm. While the inmate was being given in-cell decontamination instructions, he was handcuffed and escorted to the CTC.

March 21 - Two inmates attacked another inmate on the Facility A Main Exercise Yard. Staff fired Exact Impact rounds in an effort to gain compliance to stop the attack. As staff were responding to the incident, the inmates stopped the attack and got down on the ground. The victim inmate received nine puncture wounds and eight lacerations. There were no staff injuries as a result of the incident.

March 19 - Two inmates attacked another inmate on the Main Exercise Yard of Facility B during yard recall. To gain compliance, staff used three Exact Impact rounds, OC pepper spray, physical force, and Expandable Batons. Two of the Exact Impact rounds hit their intended targets; the third round missed the intended target, but did not strike any inmates. All inmates involved had minor injuries. No staff injuries occurred as a result of the incident.

March 14 - In Unit 2A, two inmates were being escorted back to their cell following showers. Both inmates were placed inside their cell and when the handcuffs were removed from the first inmate, he attacked the second inmate with a razor blade, who was still in handcuffs. Staff immediately gave orders to stop the attack; to which the inmate did not comply. OC pepper spray was used and the inmate stopped the attack. The victim inmate was transported to the CTC, then to a hospital for additional treatment. The assailant was de-contaminated and re-housed. No staff were injured as a result of the incident.

March 13 - The Facility B8 Floor Officer was inside the Housing Unit office when a BMU inmate entered the office and proceeded to physically assault the Officer. The inmate hit the officer twice in the face. Staff used physical force and OC pepper spray to gain control of the combative inmate. The inmate was escorted from the area, decontaminated and found to have no injuries and placed in Administrative Segregation. The Officer was transported to the hospital for medical evaluation. Upon completion of the evaluation, the Officer returned to PBSP.

March 12 - Five inmates attacked two inmates in the dayroom. Staff orders to stop were ignored. Three Exact Impact Rounds were used to gain compliance. All inmates sustained minor injuries; there were no staff injuries as a result of the incident.

March 6 - On the exercise yard, two inmates were involved in mutual combat. Three Exact Impact rounds were fired in an attempt to gain compliance with orders to stop fighting and get down. The inmates stopped fighting and were placed in handcuffs and re-housed.

March 5 - Two cellmates were involved in mutual combat on the tier. The inmates ignored orders to stop fighting and get down on the floor. Staff fired one 40mm Exact round to gain compliance from the inmates. The round missed the inmates; however it had the desired effect and the inmates stopped fighting, got down on the floor and were handcuffed by responding staff. The inmates were medically examined and re-housed.


February 2008


February 20 - During a controlled released, three inmates were involved in a fight. Staff used OC pepper spray to quell the incident. One inmate manufactured weapon was discovered. All inmates were exposed to OC and one inmate received minor injuries to his head. The other inmates were not injured. All involved inmates were re-housed in ASU. There were no staff injuries.

February 4 - An Administrative Segregation inmate being returned to his cell spit on the escorting officer. The officer was struck on his face.

February 1 - An inmate leaving the showers turned and kicked one of the escorting officers in the leg. Staff immediately gained control of the inmate by placing him on the floor. The inmate was medically evaluated and returned to his cell without further incident. No officers were injured.


January 2008


January 23 - Three inmates were involved in a fight on the main yard of Facility A. Chemical agents and one exact impact round were used to stop the fight.

January 18 - Over 60 northern and southern Hispanic inmates were involved in a riot on Facility B. The riot occurred simultaneously in Education, the B-2 Dining Room, and the Main Yard. One Mini-14 round was fired as a warning shot. Exact impact rounds and OC were also used to quell the riot. Two inmates were transported by ambulance to outside hospitals although their injuries were not considered life threatening. Two staff members also suffered minor injuries.

January 15 - An inmate struck an officer in the face while being prepared for transport to the Department of Mental Health. Additional staff used an expandable baton and pepper spray to gain control of the inmate. The assaulted officer suffered some swelling and a cut on the nose.
