April 28 - California Correctional Institution - A Correctional Officer went into IVA Housing Unit 1 to investigate noise coming from a cell. The Officer observed what appeared to be an inmate committing battery on another inmate. The Officer activated his personal alarm and utilized his MK-9 fogger to stop the battery. Upon medical evaluation, the victim inmate notified the on-duty physician that the inmate assailant attempted to sexually assault him. Both inmates were transported to an outside medical facility. CLICK HERE for CCI archive. April 26 - California Correctional Institution - A Code 1 alarm was announced outside the Unit II Dining Hall #1. Responding staff utilized O.C. pepper spray to stop the attempted murder of an inmate with an inmate manufactured weapon. Two inmates were suspected assailants on another inmate. All three inmates were placed in restraints. Medical staff responded and provided aid to the victim inmate who was ultimately sent to an outside hospital. A razor blade attached to a pencil with string and tightly rolled up aluminum from a soda can sharpened to a point was collected into evidence. April 26 - California Correctional Institution -In Housing Unit #6, Officers were picking up trays after the evening meal, when an inmate stuck an Officer's arm through the open food port and holding what appeared to be a weapon, started to move his arm in a side to side slashing motion. The Officer utilized his O.C. pepper spray through the food port. The inmate was placed in restraints and removed from his cell. The inmate was decontaminated and placed in a holding cell where he was medically evaluated and eventually re-housed without further incident. No staff injuries were reported. CLICK HERE for CCI archive. April 25 - California Correctional Center - The inmate was initially transported to Banner Lassen Hospital for medical treatment and evaluation of a reported problem. The inmate was in the emergency room awaiting test results when the inmate became verbally abusive and attempted to leave. An inmate became assaultive and battered an Officer and attempted to batter Emergency Room Registered Nurses. CLICK HERE for CCC archive. April 21 - California Correctional Institution - Staff were picking up trays in A Section after count cleared in Housing Unit #2. An inmate's food port was open to collect the food tray, when the inmate sexually assaulted the Officer. The Officer drew their MK9X fogger and sprayed the inmate through the food port and the inmate stopped the sexual assault. The inmate was removed from the cell, decontaminated and medically evaluated. CLICK HERE for CCI archive. April 21 - California Correctional Institution - While being escorted to clinic holding, an inmate attempted to spit in the face of the escorting Officer. Staff utilized O.C. pepper spray, expandable batons and physical force to stop the inmate. The inmate received abrasion to the face and bruises to both legs and shoulder. April 21 - California Correctional Institution - While conducting cell searches in Housing Unit 4A-7C, an inmate was seen trying to flush possible contraband down the toilet in his cell. The Officer at the door ordered the inmate to stop and cuff up. The inmate refused the orders. Staff utilized their MK-9 fogger to gain compliance and stop the inmate from flushing the contraband. The inmate complied with staff orders and was removed from the cell, decontaminated and medically evaluated. CLICK HERE for CCI archive. April 19 - California Correctional Institution - While attempting to remove an inmate from his cell, the inmate took two rapid steps towards staff in a threatening manner. Staff utilized O.C. pepper spray to stop the threat and close the cell door. The inmate was ordered to submit to restraints but refused. O.C. pepper spray was utilized again to gain compliance. The inmate then complied with staff orders and was placed into restraints. The inmate was decontaminated and medically evaluated. CLICK HERE for CCI archive. April 18 - California Correctional Institution - After receiving a shower, an inmate refused to return to his cell. Staff gave several verbal orders to the inmate to return to his cell. The inmate attempted to come down the stairs of the housing unit, as staff utilized OC pepper spray. The inmate complied with orders to lie in a prone position and he was placed in handcuffs. The inmate was decontaminated and received medical treatment. CLICK HERE for CCI archive. April 18 - High Desert State Prison - An inmate approached the dining hall without a state issued blue shirt. Staff informed the inmate he would not be allowed into the dining area without a shirt. The inmate disregarded staff and continued walking into the dining area. Staff approached the inmate and instructed him to submit to handcuffs. The inmate refused, staff then utilized OC pepper spray. After being sprayed, the inmate charged the Officers; one of the Officers slipped in the pepper spray and fell to the ground. The inmate began kicking the Officer in the chest. Responding staff restrained the inmate by utilizing batons. The Facility B inmates that were on the yard became disruptive during the incident by yelling and some refused to get down on the ground. Code Three responders arrived and all inmates on the yard were placed into restraints. Facility B will be placed on modified program pending an investigation into the incident. The Facility Sergeant received a possible broken hand; one staff was struck in the head with a baton and received a large contusion to the head. A third Officer injured his back responding to the incident. The three Officers were transported to an outside hospital for treatment. The inmate received abrasions and was placed in ASU. April 17 - Parole - A Parole Agent stopping to allow a pedestrian to cross a Santa Ana street was attacked by this same pedestrian, later identified as a parolee. The parolee dragged the Parole Agent out of the car and began to beat the Agent. The Agent used his OC pepper spray on the parolee but it had no effect. The Parole Agent began to fight for his life but was unable to reach his firearm. During the ensuing attack, the Agent's neck-chain badge was clearly visible, and he identified himself, asking a gathering crowd to call the police. However, the crowd encouraged the attacker. The assault is estimated to have lasted 10 minutes. A single car was sent to the scene as a result of the Santa Ana Police responding to a 911 hang-up. With the Officer's help, the Parole Agent was able to gain control over the parolee who was booked and charged with assault on a Peace Officer. April 14 - Folsom State Prison - Staff used a baton to stop an inmate who was striking another inmate in the head and torso with a wooden cane. During the initial investigation it was discovered the suspect had been stabbed earlier the same morning. CLICK HERE for Folsom State Prison archive. April 13 - California Rehabilitation Center - Two inmates battered another inmate. Responding staff used OC pepper spray to stop the attack. The two inmates were transferred to California Institution for Men. The third inmate was placed on contraband watch and is pending transfer to CIM. No staff injuries were reported. CLICK HERE for California Rehabilitation Center archive. April 13 - California State Prison, Sacramento - Two inmates were involved in mutual combat on the basketball court of A Facility Main Yard. Officers ordered the combatants to stop fighting, to which the combatants did not comply. The Officers utilized OC pepper spray. The Sergeant responded and ordered one of the inmates to stop fighting; which he did not comply. The Sergeant utilized his expandable baton and struck one of the inmates. Both inmates stopped fighting and got down in a prone position on the ground. The inmates were decontaminated and medically evaluated. One inmate received abrasions to the shoulder, back and knees. April 13 - California State Prison, Los Angles County - An inmate committed battery on two Officers causing bleeding from the nose and redness to the other Officers face. Responding staff used OC pepper spray and physical force to stop the incident. All staff were treated and released. CLICK HERE for CSP-LAC archive. April 13 - California State Prison, Los Angeles County - While conducting clothed body searches in Facility B, an inmate turned and began punching an Officer in the face. Another Officer began to help the Officer but was also attacked by the assailant. The first Officer sustained facial injuries and was treated at an outside medical facility. April 13 - High Desert State Prison - 10 to 15 black inmates engaged in a riot on the Facility D lower yard. Staff used OC pepper spray and two mini 14 warning shots to stop the melee. There were no staff injuries and no serious injuries to the inmates. One inmate manufactured weapon was discovered. The black population will be placed on modified program pending investigation into this incident. The inmates involved in the riot were re-housed in ASU. CLICK HERE for High Desert State Prison archive. April 13 - R.J. Donovan Correctional Facility - An inmate threw an unknown liquid substance on an Officer. The inmate was placed in ASU. CLICK HERE for RJD Correctional Facility archive. April 12 - Avenal State Prison - A riot occurred involving seven black inmates and seven white inmates. The inmates refused staff's orders to stop fighting, so OC pepper spray was used. The unit was placed on modified program pending searches and interviews. The involved inmates were placed in ASU. CLICK HERE for Avenal State Prison archive. April 12 - California State Prison, Sacramento - Two inmates engaged in mutual combat on the second tier of section A. The Officer activated his alarm and ordered the inmates to stop fighting; to which the inmates did not comply. The Officer shot a baton round at the combatants and missed, striking the wall to the side of them. Responding staff arrived and ordered the inmates to stop, but they still refused. Staff utilized OC pepper spray and the inmates stopped fighting. The inmates were decontaminated and medically evaluated. The inmates sustained abrasions to the chest, back, arm and legs. Both inmates were placed in ASU. CLICK HERE for CSP-Sacramento archive. April 12 - North Kern State Prison - Three inmates assaulted two other inmates during the evening meal. An impact round was used to quell the incident. One of the inmates was discovered with a weapon. All involved inmates were medically evaluated and re-housed without incident. April 10 - California State Prison, Sacramento - A riot broke out on the Minimum Support Facility yard involving four black inmates and seven northern Hispanic inmates. Staff utilized OC pepper spray to quell the incident. One staff member was treated for OC pepper spray exposure. Several inmates received minor scrapes and abrasions. CLICK HERE for CSP-Sacramento archive. April 10 - Centinela State Prison - An inmate was attacked by three other inmates. The victim inmate received several puncture wounds and slashes. April 10 - High Desert State Prison - Two inmates engaged in mutual combat on Facility D yard. Staff gave verbal orders for the inmates to get down in a prone position; to which the inmates did not comply. Staff utilized three pocket grenades and six direct impact rounds to quell the incident. Four of the six rounds struck its intended target. The two inmates sustained injuries consistent with being involved in mutual combat. Both inmates were re-housed in their assigned housing units. April 9 - California State Prison, Sacramento - While staff were conducting showers on A Facility 3-Block PSU, an officer was escorting an inmate down the stairs from the upper tier. As they descended the stairs the inmate began to rush down the stairs and jumped toward the Officer who was standing on the lower tier. The inmate kicked the Officer and lost his footing and fell to the ground. The inmate then stood back up and rushed back towards the Officer. The Officer ordered the inmate to get down, pulled his pepper spray and administered it to the inmate's face. The inmate continued to rush towards the Officer, so the Officer jumped down the lower stairs and onto the dayroom floor, then administered another burst of pepper spray into the face and upper torso of the inmate. Responding staff grabbed the inmate's back area and directed him to the floor in a prone position. No significant injuries to staff occurred because of this incident. April 9 - Corcoran State Prison - On 4A Facility Group Exercise Yard #2, an Officer observed an inmate turn and strike another inmate in the face and torso in what appeared to be a stabbing motion. The victim inmate fought back in an attempt to defend himself. During the struggle, the assailant dropped unidentified contraband and the victim inmate bent down and appeared to pick up the item. The victim inmate then began using a stabbing motion towards the inmate. The Officer gave verbal orders to stop with negative results. Three direct impact 40 mm rounds were fired. Both inmates then complied with orders to stop. The inmates were removed from the yard without further incident and medically evaluated. Both inmates were found to have sustained puncture wounds. April 9 - Kern Valley State Prison - Two inmates became engaged in mutual combat on the Facility A lower exercise yard. Staff gave verbal orders for the inmate to stop fighting and to assume a prone position on the ground. The inmates ignored staff orders and continued to fight. The FAB3 Control Booth Officer discharged one direct impact round from his 40mm launcher at the combatants. The inmate stopped fighting and assumed a prone position on the ground. Responding staff placed the inmates in handcuffs and escorted them to the program area where they were medically cleared and subsequently re-housed in ASU. April 9 - Salinas Valley State Prison - Facility C staff observed two inmates fighting in a housing unit. The inmates failed to comply with staff orders to stop fighting and a Code 1 response was initiated. Responding staff utilized OC pepper spray to gain compliance. A search of the immediate area resulted in the discovery of a stabbing type weapon. Both inmates were medically evaluated and decontaminated. One of the inmates suffered several puncture wounds, which required further evaluation at an outside medical facility and the inmate later returned to the institution. April 7 - California State Prison, Sacramento - On the C-Facility General Population yard, two Crip inmates committed battery on a third Crip inmate. Staff used a 1040 Blast grenade and pepper spray to quell the incident. No serious injuries occurred as a result of this incident. CLICK HERE for CSP-Sacramento archive. April 7 - California State Prison, Sacramento - On the C-Facility General Population yard, two Blood inmates committed battery upon a third Blood inmate. April 7 - Kern Valley State Prison - Two inmates committed battery on another inmate in the dayroom of FAB8. The Control Booth Officer observed two inmates run towards another inmate and began striking him in his face and upper torso. Staff gave several verbal orders for the inmates to stop fighting and assume a prone position on the ground; the inmates ignored staff orders and continued fighting. The Control Booth Officer discharged one direct impact round at the assailants but did not hit his intended target. Responding staff gave verbal orders for the inmates to stop fighting; the inmates stopped fighting and assumed a prone position on the ground. The inmates were placed into handcuffs and escorted to the program where they were medically cleared and re-housed in ASU. There were no staff injuries. CLICK HERE for Kern Valley State Prison archive. April 7 - Pelican Bay State Prison - A PSU inmate assaulted an Officer while he was under escort. The inmate kicked the escorting Officer and the Officer placed the inmate on the ground. The inmate was wearing leg irons and has a lengthy history of staff assaults. CLICK HERE for Pelican Bay State Prison archive. April 6 - Pelican Bay State Prison - In A Section dayroom of Unit B-6, two inmates attacked another inmate. While in the process of conducting the unit barber program, the Control Booth Officer released the victim for a haircut. A fight broke out between the inmates. The inmates refused verbal orders to stop and the control booth Officer fired one exact impact round to stop the attack. The round struck the suspect in the back. All of the inmates complied with orders to get down on the floor. The victim received a cut to the head. There were no staff injuries. All inmates were re-housed in Ad-Seg. CLICK HERE for Pelican Bay State Prison archive. April 5 - CSP, Los Angeles County - The FAB5 Floor Officer #2 was instructed to escort an AD-Seg inmate from his cell. When the Officer opened the food port to place handcuffs on the inmate, the inmate put his hands behind him for the handcuffs to be placed. As the Officer was handcuffing the inmate, the inmate turned and grabbed the Officer's left wrist just past his watch and pulled the Officer's hand and forearm into the cell using the Officer's watch as leverage. O.C. pepper spray was utilized and the inmate released his grip on the Officer's arm. The Officer was evaluated by on-site medical staff, no injuries were reported. CLICK HERE for CSP-LAC archive. April 4 - California Institution for Men - An altercation between Mexican inmates and white inmates occurred within the Laguna Housing Unit on the West Yard. The incident was contained within 15 to 20 minutes and the unit was totally secured within an hour. There were no staff injuries as a result of the incident other than exposure to chemical agents used to gain control of the melee. Prior to the incident, the Institution was on lockdown because of the Tehachapi incident the day before. As a result, response time was shortened significantly for staff that responded from other yards. CLICK HERE for CIM archive. April 4 - California Institution for Men - Five Southern Hispanic inmates attacked white inmates. OC pepper spray was used to quell the incident. Seven inmates were taken to triage and treatment area for medical attention, and five inmates were transported to outside hospitals. Two of the five inmates sustained stabbing-type injuries. April 3 - California Correctional Institution - Two Southern Hispanic inmates rushed staff in a program office on the maximum-security, general population yard and attacked staff with inmate manufactured stabbing weapons. OC, batons and physical force were used to subdue the inmates. Two Sergeants and two Officers were sent to outside medical facilities for treatment of lacerations, stab and puncture wounds. One Sergeant and one Officer were treated and released. The other Sergeant was placed in intensive care for treatment of a skull fracture. A fourth Officer was hospitalized for injuries she sustained to her knee when responding to the incident. Both inmates were taken to outside hospitals for treatment, and were subsequently released and transferred to Corcoran State Prison's Administrative Segregation Unit. Two inmate weapons were recovered at the crime scene. The prison was on lockdown and visiting canceled for the weekend. CLICK HERE for CCI archive. April 2 - CSP, Los Angeles County - During the morning meal, an Officer ordered a disruptive inmate to leave the dining area for counseling. In the process of counseling the inmate, the inmate struck the Officer in the face. While the Officer was disoriented, the inmate continued to assault the Officer by repeatedly striking him in the face. The Officer sustained contusions and abrasions to his face and head. April 2 - CSP, Los Angeles County - On the North Exercise Yard, an Officer ordered an inmate to submit to being placed in handcuffs and the inmate refused.
