February 28 - Avenal State Prison - An inmate was discovered in possession of a controlled substance. As the inmate was being escorted to a support building, he grabbed the substance and attempted to exit the office. Staff attempted to gain control of the inmate who did not comply with staff orders to cease. The inmate then kicked one Sergeant in the head. The inmate was placed in ASU and the substance was processed into evidence.

February 27 - Centinella State Prison - While applying restraints to a disruptive inmate, an Officer was struck in the head. The Officer suffered bruising and swelling and was taken for outside medical evaluation and treatment. The inmate was medically cleared and place in ASU.

February 27 - High Desert State Prison - Two inmates assaulted another inmate. Staff used OC pepper spray to stop the inmates from fighting. All three inmates were placed in ASU. The attacked inmate was taken to an outside hospital for treatment of his injuries.

February 25 - California State Prison, Sacramento - While on the A Facility Small Side Yard, an inmate swung at an Officer. The Officer turned to the left causing the inmate to miss his face and striking him in the back shoulder area. The inmate took another swing, which struck the Officer on the back. The inmate continued his attack as he was being sprayed with OC pepper spray. The inmate and the Officer fell to the ground. The inmate refused to comply with staff orders to stay down on the ground. As the inmate was attempting to stand up, staff struck him once in the shoulder with a forward strike from an expandable baton. The inmate remained on the ground and submitted to restraints without further incident. The Officer sustained no visible injuries, but complained of some pain. The inmate sustained a wound to the facial area and swelling to the shoulder.

February 24 - Corcoran State Prison - During recreation, a riot occurred when a Fresno Bulldog inmate was released from a housing unit to walk to the visiting gate. Just outside the front door of the housing unit, 16 Southern Hispanic inmates and one white inmate rushed from the basketball court and attacked the inmate. Staff responded and quickly quelled the incident utilizing OC pepper spray. Three inmates suffered only minor injuries.

February 24 - California State Prison, Sacramento - Staff observed an inmate walking down the tier and being pushed inside a cell. The inmate backed out of the cell attempting to fend off an attack by another inmate. After exiting the cell, another inmate joined in on the assault. The unit alarm was sounded and the staff processing inmates to the yard immediately responded to the attack. As staff entered the unit and gave orders to stop the attack, the two inmates complied. Responding staff observed one of the inmates dropping an inmate manufactured weapon from the second tier onto the first tier. The weapon was fashioned from round metal stock, possibly oven rack material, measuring approximately 8 inches long by 1/4 inch in diameter. No force was used by staff. The inmate victim was transported to UC Davis Hospital.

February 23 - CSP-Lancaster - An inmate in Facility C, Building 1 was exiting his cell for a cell search, he became agitated and struck an Officer on the head. Injuries to the Officer consisted of bruises to the head, upper body and shoulder. The Officer was seen at High Desert Medical Group.

February 22 - Central California Women's Facility - An inmate housed in Administrative Segregation battered a Correctional Officer with an unknown liquid through the food port. The liquid struck the Officer in the face and chest area.

February 20 - Kern Valley State Prison - During evening meal, an Officer observed two inmates involved in mutual combat. The Officer ordered the inmates to stop fighting with negative results. The Officer used OC pepper spray striking both inmates. The inmates stopped fighting and assumed a prone position. Responding staff placed the inmates in handcuffs with no further incident. Both inmates were medically cleared and re-housed.

February 20 - Pelican Bay State Prison - During a controlled released, three inmates were involved in a fight. Staff used OC pepper spray to quell the incident. One inmate manufactured weapon was discovered. All inmates were exposed to OC and one inmate received minor injuries to his head. The other inmates were not injured. All involved inmates were re-housed in ASU. There were no staff injuries.

February 18 - Corcoran State Prison - Two general population inmates attacked another inmate with a weapon. The Yard Observation Officer observed an inmate walk up behind another inmate and began to strike him in the head. Another inmate approached the victim from the front and began striking him. The Control Booth Officer gave verbal orders for the inmates to stop the attack with negative results. The Control Booth Officer then fired one round; which stopped the fighting. When one of the assailants assumed a prone position, the Observation Officer noticed a weapon in the inmate's hand. The officer maintained visual surveillance on the inmate as the victim was removed from the yard. The inmate combatants were escorted to holding cells to be medically evaluated and re-housed.

February 18 - Folsom State Prison - While feeding breakfast a fight between Black and Southern Hispanic inmates broke out, quickly escalating into a riot. 180 inmates were involved in the melee. Staff stopped the riot, which lasted approximately seven to ten minutes.

February 16 - California Institution for Women - An Officer observed two inmates striking each other in the head with closed fists. The Officer activated her personal alarm while giving verbal orders to stop fighting with negative results. The Officer then used OC pepper spray, hitting one of the inmates. The Officer then ordered the other inmate to get down, to which she complied.

February 15 - Corcoran State Prison - During the evening meal release, two Correctional Officers observed an inmate on top of another inmate holding him by the throat and striking him repeatedly to the head with an object later identified as a sock containing "D" size batteries. The Control Booth officer activated his alarm. Both inmates were ordered to stop with negative results. One Officer used his expandable baton striking the inmate with negative results. The inmate was given a second direct order to stop fighting but the inmate did not comply and continued to attack the inmate. The Officer again used his baton striking the inmate. This stopped the inmate from his continued attack. Responding staff placed both inmates in restraints without further incident.

February 15 - Kern Valley State Prison - Two Southern Hispanic inmates committed battery on another Southern Hispanic inmate on the lower exercise yard. The observation Officer fired two direct impact rounds from his 40mm launcher; the rounds did not hit any inmates but had the desired effect of stopping the attack. All inmates were medically cleared and re-housed in ASU.

February 15 - Kern Valley State Prison - Two inmates were involved in mutual combat. The Control Booth Officer fired one direct impact round from his 40mm launcher; the round did not strike any inmates but stopped the fighting and the inmates assumed a prone position. The inmates were medically cleared and re-housed.

February 15 - Avenal State Prison - As inmates were being processed to work outside the fence 14 Southern Hispanics and six Bulldogs were involved in a riot. The inmates refused to obey staff ordered to stop, resulting in staff's use of force. Two 40mm stinger rounds and two grenades were used to stop the melee. No staff injuries or serious inmate injuries occurred.

February 14 - California State Prison, Sacramento - An inmate approached another inmate in a joking manner and was observed placing his arm around the victim's shoulders before making a slicing motion to the victim's neck. The inmate fell to the ground with blood spewing from his neck. The assailant dropped the weapon, stepped away, and got down on the ground. The alarm was sounded and the assailant restrained as responding staff immediately started life saving measures on the victim. The victim was transported to Folsom Mercy Hospital where he was pronounced dead.

February 14 - High Desert State Prison - Two inmates attacked another inmate with an inmate manufactured weapon during afternoon yard activities. The combative inmates complied with staff ordered to get down. Therefore no force was utilized. The inmate manufactured weapon was made from flat metal stock measuring approximately 5-and-a-quarter inches in length and sharpened to a point at the end. The victim was medically evaluated and transported to the Correctional Treatment Center for further treatment. The two inmate assailants were re-housed in ASU.

February 14 - High Desert State Prison - Two inmates engaged in mutual combat during morning yard activities. Staff utilized one 40mm Exact Impact round to quell the incident. Both inmates were medically evaluated; one with no injuries and the other inmate sustained minor injuries.

February 13 - Avenal State Prison - A riot occurred near the vehicle sally port involving three Southerners and three Bulldogs. Pepper spray was utilized. There were no injuries to staff and no major injuries to inmates.

February 13 - Kern Valley State Prison - Two inmates in a Housing Unit engaged in mutual combat. Staff discharged one Direct Impact round from the 40mm launcher to quell the incident. No injuries were reported.

February 13 - Kern Valley State Prison - An inmate who was disruptive in a vocational area was ordered back to his housing unit where he then swung at a Sergeant. The Sergeant put his hands up to defend himself from the attack and sustained two lacerations to his hand. Staff used OC pepper spray to subdue the inmate. The inmate was placed into restraints; while conducting an unclothed body search, staff discovered a 6 to 6-and-a-half inch inmate made weapon secured to the inside of the inmate's thigh.

February 13 - California State Prison, Sacramento - Two officers ordered an inmate porter not performing his duties to lockup. The inmate ignored the order and punched one of the officers in the face as she attempted to handcuff him, knocking her unconscious. The inmate then rushed the other officer and began to strike him repeated in the head. The officer was able to grab the inmate and force him to the ground. The inmate continued to strike the officer until responding staff arrived and gained control of the inmate. The inmate was medically evaluated and sustained abrasions and pain to his knee and ankle. The inmate was placed into ASU for Battery on a Peace Officer. The female officer was taken to the hospital for evaluation. The other officer suffered a red, swollen and bruised eye.

February 12 - Salinas Valley State Prison - While an MTA was escorting an inmate to a shower, the inmate suddenly kicked the MTA in the lower leg. The inmate continued to resist the MTA as efforts were made to gain control of the inmate. Two other MTAs' responded to the area. Control of the inmate was gained by the MTAs' utilized strength and body weight to force the inmate to the ground. The inmate was medically evaluated and returned to his cell. The three MTAs' sustained minor injuries and were transported to an outside medical facility for evaluation and treatment.

February 11 - Avenal State Prison - An inmate struck a Correctional Officer on the side of his head. Staff utilized pepper spray after the inmate did not comply with orders. The officer was sent to Coalinga Regional Medical Center.

February 11 - California State Prison, Sacramento - A racial melee occurred on the main exercise yard involving nine Hispanic and Black inmates. One 40mm Direct Impact Round and OC pepper spray was used to quell the incident.

February 11 - California State Prison, Sacramento - Two Southern Hispanic inmates began to assault and stab a Black inmate with a 6 inch metal rod. Two 40mm Direct Impact Rounds, two Mini-14 warning shots, and side handle baton was used to quell the incident. The victim received numerous superficial puncture wounds and was treated on site.

February 9 - Central California Women's Facility - Two inmates on the ASU exercise yard began to engage in mutual combat. Staff's verbal commands were ignored and the inmates continued to fight. Staff utilized an OC grenade, three 37 mm multiple baton rounds and a CN grenade to stop the fighting.

February 8 - High Desert State Prison - During dayroom activities, nine 415/Crip inmates became involved in a riot. Staff utilized thirteen 40 mm Direct Impact rounds, four Stinger rounds and OC pepper spray to end the disturbance. One inmate sustained multiple head injuries and was transported to an outside hospital for treatment. All other inmates involved sustained minor injuries. All nine inmates were re-housed in Administrative Segregation.

February 7 - Corcoran State Prison - Two officers running Facility III-A, Administrative Segregation shower program began preparing to remove an inmate from his cell. The inmate was handcuffed and the Control Booth officer opened the door. The inmate slipped his right hand out of the handcuff and revealed an inmate manufactured stabbing weapon, stabbing an officer over his eye. The officer fell to the ground and the inmate continued to attack the officer. The inmate attempted to continue to stab the officer but the officer was able to grab the inmate's arm controlling the weapon. The other officer attempted to gain control of the inmate using physical force but was unsuccessful. The officer then removed her MK-9 and sprayed the inmate, which was ineffective, and the downed officer continued to struggle with the inmate. Staff in the vicinity at the time ran to stop the attack.

February 7 - North Kern State Prison - Approximately 12 inmates were involved in a riot between Northern Hispanics and Black inmates. All inmates' injuries were superficial and there were no staff injuries as a result of the incident. Three inmates were re-housed in Administrative Segregation.

February 7 - California State Prison, Sacramento - A Black inmate struck an Asian inmate on the main exercise yard. Immediately 55 Crip gang members and Asian inmates ran to engage in the melee. Code Three Response was immediately called. Two 40 mm rounds were fired and OC grenades were used to stop the incident. Two Black inmates and one Asian inmate were transported to an outside hospital for blunt force trauma to the head.

February 6 - Salinas Valley State Prison - While an officer was issuing a property receipt; an inmate suddenly grabbed the officer's arm and pulled him into his cell. Responding staff utilized pepper spray to gain control. The inmate was removed from the cell, medically evaluated and decontaminated prior to re-housing. The officer was medically evaluated and treated for injuries.

February 6 - California State Prison, Sacramento - Four inmates in the gym/dorm were involved in a fight. Staff used OC pepper spray and a baton to gain control of the incident. One of the inmates was transferred to an outside hospital. A search of the dorm resulted in four inmates possessing weapons.

February 5 - Salinas Valley State Prison - As he was being escorted to a medical examination, an inmate suddenly kicked a registered nurse. Staff used physical force to regain control of the inmate.

February 5 - California State Prison-Sacramento - Two white inmates assaulted and stabbed a third white inmate while on a main yard. The victim was transported to an outside hospital Code 3 for multiple stab wounds.

February 4 - California Men's Colony - An inmate being escorted to the showers, struck one of officers in the eye. The inmate was handcuffed and re-housed in his hospital cell on ASU status. The officer had swelling and bruising to his right eye.

February 4 - Pelican Bay State Prison - An Administrative Segregation inmate being returned to his cell spit on the escorting officer. The officer was struck on his face.

February 4 - California Institution for Men - As a nurse was giving an inmate prescribed medication, the inmate hit an officer in the face. Staff used physical force and holds to regain control of the inmate. The officer was instructed to report for a medical evaluation.

February 4 - San Quentin State Prison - While an inmate was being escorted down the tier; the inmate spit on an officer and kicked another. Both officers wrestled the inmate to the ground. One of the officers sustained a broken wrist.

February 4 - California State Prison-Sacramento - An inmate being served a CDCR 115 became disruptive and the floor officer ordered the inmate to return to his cell. The inmate moved toward the officer, pushed him and then struck him in the face with his fist. The officer responded by grabbing the inmate's arm with both hands trying to force him to the ground. The inmate continued to strike the officer as staff activated the unit alarm and attempted to pull the inmate away from the officer. Staff was not able to stop the inmate's assault. Additional responding staff arrived and was able to pull the inmate to the ground and restrain him. The inmate sustained abrasions to the ankles, bruises to the mouth, wrist, and shoulder blade. The officer sustained abrasions on his nose, side of his jaw, elbow, and his head. The officer was transported to an outside hospital for further evaluation.

February 1 - Centinela State Prison - An inmate exiting the chow hall with a cup of coffee was ordered to pour out the coffee. The inmate refused and began to argue with the officer. The inmate struck the officer in the jaw and face area. A second and third inmate joined the attack. The Sergeant and several officers fought the attackers. A fourth inmate attempting to join the attack was stopped by pepper spray. Responding staff took control of the inmates. Several officers were injured. One Sergeant suffered shoulder, back and neck pain. The first officer suffered an eye injury with blurred vision. A second officer has back and knee pain. A third officer has injuries to his elbows and head. The fourth officer has a sore jaw and bumps to the head. The three inmates were re-housed to the Administrative Segregation Unit.

February 1 - California Institution for Men - A newly arriving inmate was getting off the bus and attempted to split in an officer's face, requiring two officers to use pepper spray to subdue and cease his attempts to continue spitting. The inmate was re-housed in the Administrative Segregation Unit.

February 1 - Pelican Bay State Prison - An inmate leaving the showers turned and kicked one of the escorting officers in the leg. Staff immediately gained control of the inmate by placing him on the floor. The inmate was medically evaluated and returned to his cell without further incident. No officers were injured.
